Angelmannin oireyhtymästä kärsivien aikuisten ja lasten pitkittäinen luonnontieteellinen tutkimus
Tämän havainnointitutkimuksen tavoitteena on saada tietoa Angelmanin oireyhtymän (AS) luonnollisesta etenemisestä lapsilla ja aikuisilla, joilla on vahvistettu AS:n geneettinen diagnoosi. Tutkimuksen pääkysymykset ovat:
- Miten kehitystaidot, kuten viestintä, motoriset taidot ja sopeutumiskyvyt, muuttuvat 1 vuoden aikana AS-potilailla?
- Onko aivojen toiminnassa tai unessa tiettyjä kuvioita, jotka liittyvät AS-oireiden muutoksiin ajan myötä?
Osallistujat:
- Vierailevat tutkimuspaikalla 5 kertaa 1 vuoden aikana (noin joka 3. kuukausi) arviointeja varten.
- Suorittavat kehitykseen, käyttäytymiseen ja uneen liittyviä testejä ja kyselylomakkeita huoltajiensa avulla.
- Suorittavat elektroenkefalografiat (EEG) aivojen toiminnan mittaamiseksi ja käyttävät kotona unenseurantalaitetta (aktigrafiatietojen keräämiseksi).

Yksityiskohtainen kuvaus
- Longitudinaalinen, luonnollinen seurantatutkimus 40 AS-potilaasta, jossa on erityisesti nuoria osallistujia.
- Pyritään luomaan normatiivista tietoa, joka auttaa tulevien AS-kliinisten tutkimusten suunnittelussa ja tulosten tulkinnassa sekä tukee tarkempien diagnoosityökalujen ja kohdennettujen terapioiden kehittämistä AS:lle.
- Adaptiivista käyttäytymistä, viestintää, haastavaa käyttäytymistä, yleistä terveyttä, sairauden kokonaisvaikeusastetta ja yleisiä samanaikaisia sairauksia (mukaan lukien epilepsia ja unihäiriöt) seurataan standardoiduilla arvioinneilla.
- Biomarkkeritietoja, jotka kerätään EEG:llä ja aktigrafialla, analysoidaan arvioitaessa ikään liittyviä muutoksia kliinisissä tuloksissa ja neurofysiologisissa mittareissa.
- Osallistujat osallistuvat enintään viiteen henkilökohtaiseen tutkimuskäyntiin 12 kuukauden aikana: viikko 0, viikko 13, viikko 26, viikko 39, viikko 52.

Yksilöt, jotka ovat vähintään 1 vuoden ikäisiä ja joilla on geneettisesti vahvistettu AS-diagnoosi.
Kuvaus
Osallistumiskriteerit:
- Osallistujalla on ensisijainen kliininen Angelmanin oireyhtymän diagnoosi, jossa on dokumentoitu geneettinen variaatio, joka vaikuttaa UBE3A-geenin toimintaan ihmisen 15q11.2-q13.3 lokuksessa. Samaan aikaan esiintyvät sairaudet (esim. autismikirjon häiriöt, CP-vamma, kehitysvamma) ovat sallittuja; kuitenkin Angelmanin oireyhtymän on oltava ensisijainen kliininen diagnoosi.
- Osallistuja on mies- tai naispuolinen (syntymässä määritetty sukupuoli) ja vähintään 1 vuoden ikäinen ensimmäisessä tutkimuskäynnissä.
- Osallistujalla on tutkimuskumppani, joka täyttää alla olevat tutkimuskumppanin kriteerit.
- Osallistujalla, jos hän ei kykene antamaan tietoista suostumusta, on sopiva edustaja, joka on vähintään 18-vuotias ja halukas ja kykenevä antamaan tietoista suostumusta osallistujan puolesta nykyisten ICH-ohjeiden ja sovellettavien laitossäädösten mukaisesti.
Henkilöiden on täytettävä seuraavat kriteerit voidakseen osallistua tutkimuskumppaneina:
- Tutkimuskumppani on vanhempi tai ensisijainen hoitaja, joka on vähintään 18-vuotias.
- Tutkimuskumppanilla on jatkuvaa yhteyttä osallistujaan ja tutkijan mielestä hänellä on riittävästi tietoa osallistujan nykyisestä tilasta antaakseen tarkkoja ja ajantasaista tietoja.
- Tutkimuskumppanilla on riittävä englannin kielen taito suorittaakseen tutkimuskumppanin arvioinnit.

- Tutkimuskumppani on tutkijan mielestä luotettava ja pätevä; halukas ja kykenevä saattamaan osallistujan kaikkiin tutkimuskäynteihin ja noudattamaan tutkimusmenettelyjä; tavoitettavissa puhelimitse tai sähköpostitse tarvittaessa; ja riittävän tietoinen osallistujan nykyisestä tilasta antaakseen tarkkoja ja ajantasaista tietoja osallistujan terveydestä ja hyvinvoinnista.
Poissulkemiskriteerit:
- Osallistujalla on vähintään yksi muu tunnettu geneettinen poikkeavuus ihmisen 15q11.2-q13.3 lokuksen ulkopuolella, joka aiheuttaa todennäköisen tai tunnetun kehitysvamman.
- Vähintään yksi osallistujan käyttämä standardinhoito (lääke tai lisähoito) on muutettu 28 päivän (4 viikon) aikana ennen ensimmäistä tutkimuskäyntiä. Hoidot sisältävät muun muassa epilepsialääkkeiden annokset, käyttäytymishoitoon liittyvät lääkkeet, unilääkkeet, gabapentiini, kannabidioli, erikoisruokavaliot, ravintolisät, puheterapia, toimintaterapia, sovellettu käyttäytymisanalyysi (ABA), psykososiaaliset interventiot, fysioterapia tai ravitsemustuki.
- Osallistujalla on epävakaa epilepsia, joka määritellään hätäosastokäynniksi tai sairaalahoidoksi kouristuksiin liittyvistä syistä 28 päivän (4 viikon) aikana ennen ensimmäistä tutkimuskäyntiä.
- Osallistuja on hedelmällisessä iässä ja on joko raskaana, imettää tai ei käytä riittävää ehkäisykeinoa; pidättyminen seksistä on hyväksyttävää.
- Osallistujalla on kliinisesti merkittävä syöpähistoria; kliinisesti merkittävät poikkeavat testitulokset; kliinisesti merkittävä sydän- ja verisuonitauti, veritauti, maksatauti, lihastauti, hermostotauti tai munuaistauti; tai hän on kokenut muita kliinisiä tapahtumia, jotka tutkijan mielestä tekevät osallistumisen sopimattomaksi.
- Osallistujalla on elinaikainen hoitohistoria solu- tai geenipohjaisesta hoidosta, mukaan lukien antisense-oligonukleotidit tai geenieditointihoidot.
- Osallistuja on saanut muita kokeellisia hoitoja kuin solu- tai geenipohjaisia hoitoja 28 päivän tai 5 puoliintumisajan (kumpi on pidempi) aikana ennen ensimmäistä tutkimuskäyntiä.
- Osallistuja on tällä hetkellä mukana tai aikoo osallistua interventiotutkimukseen, jossa käytetään kokeellista lääkettä tai laitetta suunnitellun seuranta-ajanjakson aikana.
- Osallistujalla on tunnettu kontraindikaatio elektroenkefalografiaan, aktigrafiaan tai mihin tahansa muuhun arviointiaikataulussa kuvattuun tutkimusmenettelyyn.
- Osallistuja tai tutkimuskumppani on tutkijan mielestä muulla tavoin sopimaton osallistumaan, mukaan lukien kyvyttömyys täyttää tutkimuksen vaatimuksia.
