PERIOP 2 - Innocuité et efficacité de l'HBPM par rapport à la thérapie de transition avec placebo pour les patients sous warfarine à long terme nécessitant une interruption temporaire de la warfarine.
Un essai contrôlé randomisé en double aveugle comparant la thérapie de pont post-opératoire à l'héparine de bas poids moléculaire par rapport à la thérapie de pont placebo pour les patients à haut risque de thromboembolie artérielle (PERIOP 2)

Description détaillée
Il y a un nombre croissant de patients qui reçoivent un traitement à long terme par la warfarine pour la prévention de la thromboembolie artérielle. L'approche actuelle de la gestion périopératoire de l'anticoagulation (c. « thérapie de transition ») avec de l'héparine de bas poids moléculaire (HBPM) n'est pas standardisée et n'a pas été évaluée par des études randomisées adéquates. Cependant, la plupart des cliniciens recommandent une thérapie de transition.
Nous avons récemment terminé une étude pilote multicentrique à un seul bras sur la thérapie de transition HBPM. Cette étude dans 10 centres a porté sur 224 patients en 10 mois. Dans l'étude pilote, le taux d'événements thromboemboliques postopératoires était de 3,1 % et 75 % de ceux-ci sont survenus chez des patients dont l'anticoagulation avait été interrompue en raison d'un saignement.
Conception : Un essai contrôlé prospectif multicentrique randomisé en double aveugle. Patients : Patients éligibles et consentants consécutifs de 11 hôpitaux d'enseignement au Canada. Un total de 1773 patients porteurs de prothèses valvulaires cardiaques recevant une anticoagulation orale à long terme avec de la warfarine ou des patients souffrant de fibrillation/flutter auriculaire et d'un facteur de risque majeur qui nécessitent une chirurgie non cardiaque élective ou une procédure invasive nécessitant l'inversion de leur traitement anticoagulant oral.
Calendrier de traitement : le consentement sera obtenu en préopératoire, mais la randomisation sera effectuée en postopératoire après confirmation de l'admissibilité.
Période préopératoire : Chez tous les participants, le traitement par la warfarine sera interrompu cinq jours avant l'intervention. La daltéparine, une HBPM, sera administrée à raison de 200 UI/kg sc tôt le matin pendant les trois jours précédant l'intervention, mais non compris le jour de celle-ci, sauf le jour précédant l'intervention chirurgicale, la dose sera de 100 UI/kg administrée 24 heures en préopératoire. La warfarine sera reprise le soir de l'intervention.
Période postopératoire : la daltéparine ou le placebo seront administrés quotidiennement (à partir du lendemain de l'intervention), à condition que l'hémostase chirurgicale soit atteinte, et seront poursuivis pendant au moins quatre jours et jusqu'à ce que l'INR soit > 2,0. Les patients considérés comme à haut risque d'hémorragie majeure postopératoire recevront de la daltéparine ou un placebo à une dose de 5 000 UI sc par jour. Les patients qui subissent des procédures considérées comme à faible risque de complications hémorragiques reprendront la daltéparine ou un placebo à 200 UI/Kg s.c. quotidien.
Résultats : Le critère de jugement principal sera la fréquence des épisodes de thromboembolie majeure sur une période de suivi de 90 jours suivant le moment de la randomisation. Les critères de jugement secondaires incluront les saignements majeurs et la survie globale.
Pertinence : Ponter ou ne pas ponter est une question clinique courante, sans preuves d'essais randomisés pour guider les cliniciens. Cet ECR déterminera si le pontage postopératoire réduit le risque de thromboembolie ou cause des dommages.

Critère d'intégration:
- Consentement éclairé,
- Patients âgés de plus de 18 ans
- Patients porteurs d'une valve cardiaque prothétique (mécanique)
- Patients atteints de fibrillation auriculaire ou de flutter auriculaire et présentant un facteur de risque majeur (antécédents d'AIT ou d'AVC, hypertension artérielle, diabète, âgés de plus de 75 ans, dysfonctionnement modéré/sévère du ventricule gauche)
- Qui reçoivent une anticoagulation orale à long terme et nécessitent une chirurgie non cardiaque élective ou une procédure invasive avec inversion de leur traitement anticoagulant.
Critère d'exclusion:
- Preuve de saignement actif au cours des 30 derniers jours précédant l'arrêt de la warfarine.
- Numération plaquettaire <100 x 109/L.
- Chirurgie rachidienne ou neurochirurgie.
- Espérance de vie inférieure à 3 mois.
- Clairance de la créatinine calculée <30 ml/min
- Patients nécessitant une chirurgie cardiaque.
- Valves prothétiques (mécaniques) multiples ou valve Starr-Edwards ou valve prothétique (mécanique) avec antécédents d'AVC ou d'AIT
- Antécédents de thrombocytopénie induite par l'héparine (TIH)
