상세 설명
동맥혈전색전증 예방을 위해 장기간 와파린 치료를 받는 환자가 늘고 있다. 항응고제(즉, 저분자량 헤파린(LMWH)을 사용한 "가교 치료")는 표준화되지 않았으며 적절한 무작위 연구에 의해 평가되지 않았습니다. 그러나 대부분의 임상의는 가교 요법을 권장합니다.
우리는 최근에 LMWH 가교 요법에 대한 다기관 단일 암 파일럿 연구를 완료했습니다. 10개 센터에서 진행된 이 연구에서는 10개월 동안 224명의 환자가 발생했습니다. 파일럿 연구에서 수술 후 혈전색전증 발생률은 3.1%였으며 이 중 75%는 출혈로 인해 항응고제를 투여받은 환자에서 발생했습니다.
디자인: 전향적인 다기관 무작위 이중 맹검 통제 시험. 환자: 캐나다의 11개 수련 병원에서 연속으로 적격하고 동의한 환자. 총 1,773명의 인공 심장 판막 환자가 와파린으로 장기간 경구 항응고 요법을 받거나 심방 세동/조동이 있고 선택적 비심장 수술 또는 경구 항응고 요법의 역전이 필요한 침습적 시술이 필요한 주요 위험 요인이 있는 환자.
치료 일정: 수술 전에 동의를 얻지만 적격성을 확인한 후 수술 후 무작위 배정을 수행합니다.
수술 전 기간: 모든 참가자에서 시술 5일 전에 와파린 요법을 중단합니다. LMWH인 Dalteparin은 수술 전날을 제외하고 시술 3일 전 아침 일찍 200 IU/kg sc로 투여하며 수술 전날을 제외하고는 100 IU/kg을 투여합니다. 수술 전 24시간. 시술 당일 저녁에 와파린을 재개합니다.
수술 후 기간: Dalteparin 또는 위약은 외과적 지혈이 달성된 경우 매일 투여되며(시술 후 아침 시작) 최소 4일 동안 INR이 >2.0이 될 때까지 계속됩니다. 수술 후 주요 출혈 위험이 높은 것으로 간주되는 환자에게는 매일 5,000IU sc의 용량으로 달테파린 또는 위약이 제공됩니다. 출혈 합병증의 위험이 낮은 것으로 간주되는 시술을 받는 환자는 200 IU/Kg s.c에서 달테파린 또는 위약을 재개합니다. 일일.

- 인공(기계) 심장 판막 환자
- 심방세동 또는 심방조동이 있고 주요 위험인자(이전 TIA 또는 뇌졸중, 고혈압, 당뇨병, 75세 이상, 중등도/중증 좌심실 기능장애)가 있는 환자
- 장기간 경구용 항응고제를 투여받고 있으며 선택적 비심장 수술 또는 항응고제 요법을 역전시키는 침습적 시술이 필요한 환자.
제외 기준:
- 와파린을 중단하기 전 마지막 30일 이내에 활동성 출혈의 증거.
- 혈소판 수 <100 x 109/L.
- 척추 또는 신경외과.
- 수명은 3개월 미만입니다.
- 계산된 크레아티닌 청소율 <30 ml/min
- 심장 수술이 필요한 환자.
- 뇌졸중 또는 일과성 허혈 발작 병력이 있는 다중 인공(기계) 판막 또는 Starr-Edwards 판막 또는 인공(기계) 판막
