Critère d'intégration:
Participants atteints de MF qui dépendent d'une transfusion ou qui présentent une anémie symptomatique, définie comme suit :
- Anémie : une valeur d'Hb < 10 g/dL démontrée lors du dépistage enregistrée à 3 occasions distinctes avec au moins 7 jours entre les mesures (Remarque : la transfusion de globules rouges doit avoir lieu au moins 2 semaines avant la mesure de l'Hb lors du dépistage).
- Transfusion-dépendant : le participant a reçu au moins 4 unités de transfusions de globules rouges au cours des 28 jours précédant immédiatement le jour 1 du cycle 1 OU a reçu en moyenne au moins 4 unités de transfusions de globules rouges dans les 8 semaines précédant immédiatement le jour 1 du cycle 1, pour un Taux d'Hb < 8,5 g/dL, en l'absence de saignement ou d'anémie induite par le traitement. De plus, l'épisode transfusionnel le plus récent doit avoir eu lieu dans les 28 jours précédant le cycle 1 jour 1.
Score de statut de performance ECOG des éléments suivants :
- 0 ou 1 pour les étapes d'escalade de dose.
- 0, 1 ou 2 pour l'étape d'expansion de la dose.
- L'espérance de vie est supérieure à 6 mois
- Accord pour éviter une grossesse ou avoir des enfants.
- Inéligible à recevoir ou n'ayant pas répondu aux traitements disponibles pour l'anémie tels que les ASE.
- Pour TGA :
- Participants précédemment traités avec des inhibiteurs de JAK pendant au moins 12 semaines.
- Participants avec un DIPSS MF intermédiaire-2 ou élevé selon les critères de l'IWG-MRT.
- Pour le TGB :
- Les participants doivent avoir suivi un régime thérapeutique et stable de ruxolitinib pendant au moins 12 semaines consécutives précédant immédiatement la première dose du traitement à l'étude.
- Participants avec intermédiaire-1, intermédiaire-2 ou haut DIPSS MF selon les critères IWG-MRT.
Critère d'exclusion:
- Avoir subi une greffe de cellules souches allogéniques ou autologues antérieures ou être candidat à une telle greffe.
- Toute chimiothérapie antérieure, thérapie médicamenteuse immunomodulatrice, thérapie immunosuppressive, thérapie biologique, thérapie endocrinienne, thérapie ciblée, anticorps ou agent hypométhylant pour traiter la maladie du participant, à l'exception du ruxolitinib pour TGB uniquement, dans les 5 demi-vies ou 28 jours (selon la plus courte) avant la première dose du traitement à l'étude.
- Valeurs de laboratoire en dehors de la plage définie par le protocole lors du dépistage.

An adverse event (AE) was defined as any untoward medical occurrence associated with the use of a drug in humans, whether or not considered drug related.
An AE could therefore have been any unfavorable and unintended sign (including an abnormal laboratory finding), symptom, or disease (new or exacerbated) temporally associated with the use of study drug/treatment.
A TEAE was defined as any AE either reported for the first time or the worsening of a pre-existing event after the first dose of study drug until 30 days after the last dose of study drug.

Grade 3: severe or medically significant but not immediately life-threatening; hospitalization or prolongation of hospitalization indicated; disabling; limiting self-care activities of daily living.
Grade 4: life-threatening consequences; urgent treatment indicated.
Grade 5: fatal.

A DLT was defined as the occurrence of any protocol-defined toxicity occurring during the first treatment cycle, from Cycle 1 Day 1 up to and including Cycle 1 Day 28 (per regimen cycle schedule), except those with a clear alternative explanation (e.g., disease progression) or transient (≤72 hours) abnormal laboratory values without associated clinically significant signs or symptoms based on investigator determination.
The DLT-Evaluable Population included all non-backfill participants eligible for dose escalation who met the criteria outlined in the Analysis Population field.

Maximum Tolerated Dose (MTD)

The MTD was defined as the dose at which the observed DLT rate was closest to the target DLT rate of 28% using an isotonical method that took the assumption of a monotonic dose-toxicity relationship into account.
Per the protocol, the stopping rule was either (a) reaching a certain number of participants at one dose level under the early stopping rule or (b) reaching the pre-defined maximum sample size.
Dose escalation was to be considered complete only when one of these conditions was met.
After completion, the MTD was to be defined as the dose level closest to the target DLT rate.
The MTD could not be concluded until the stopping rule was met.
