Hosszú QT-szindróma-populációs genetika és szívvizsgálatok (LQTS)

HÁTTÉR:
A hosszú QT-szindróma (LQTS) egy ritkán előforduló, ismeretlen okú rendellenesség, amelyben az érintett egyének szokatlan elektrokardiográfiás repolarizációs rendellenességei vannak, és hajlamosak ájulásra és végzetes kamrai tachyarrhythmiákra. Az első LQTS családot Jervell és Lange-Nielsen írta le 1957-ben. Három hirtelen haláleset következett be négy QT-megnyúlásban szenvedő siket gyermeknél; két másik gyermek és a szülők egészségesek voltak, normális hallással és normális elektrokardiogrammal. Az eredményeket az autoszomális recesszív öröklődés mintájaként értelmezték. A későbbi jelentések normál hallású (Romano-Ward szindróma) LQTS-családokat azonosítottak, amelyek előfordulási mintája autoszomális domináns öröklődésre utal.
TERVEZÉSI NARRATÍV:
1985-től kezdődően a betegeket és nem érintett hozzátartozóikat longitudinálisan követték ebben a multicentrikus vizsgálatban, hogy széles körben alkalmazható klinikai kritériumokat dolgozzanak ki és validáljanak az életveszélyes aritmiák kockázatának rétegzésére. Genetikailag siket hallgatók populációját vizsgálták meg, hogy további családokat azonosítsanak a hosszú QT-szindróma és a veleszületett siketség egyedülálló összefüggésével, hogy bővítsék a rendellenesség recesszív formájával, a Jervell- és Lange-Nielsen-szindrómával kapcsolatos genetikai vizsgálatok adatbázisát. A betegség domináns formájának, a Romano-Ward-szindrómának az öröklődésének jobb megértése érdekében feljegyezték a kiválasztott hosszú QT-szindrómás családok törzskönyveit. Genetikai vizsgálatokat végeztek humán leukocita antigén és más fehérje markerek felhasználásával, hogy megvizsgálják a szindróma autoszomális domináns formájának génhelyét. Egy 30 betegből és 30 nem érintett hozzátartozóból álló kiválasztott csoport 24 órás Holter-monitorozást, futópad gyakorlatot, Valsalva manővert és markolat terheléses tesztet végzett annak megállapítására, hogy a betegek egyedi kardiovaszkuláris válaszreakciót váltottak-e ki az autonóm diszfunkcióra.
A tanulmányt 1993-ban újították fel. A megújulásnak hat célja volt. Az első az LQTS genetikai heterogenitását vizsgálta a Harvey-ras-1 génkapcsolat tesztelésével a meglévő, jól jellemzett LQTS családokban, szegregációs analízissel egy fő génre utalva; Azokban az LQTS családokban, amelyek nem mutattak Harvey-ras-1 kapcsolatot, más, szorosan kapcsolódó genetikai markerek keresését kezdeményezték.
