理論的根拠: 化学療法で使用される薬剤は、さまざまな方法で腫瘍細胞の分裂を止め、増殖を停止したり死滅させたりします。 放射線療法では、高エネルギー X 線を使用して腫瘍細胞に損傷を与えます。 末梢幹細胞移植により、医師はより高用量の化学療法を施し、より多くの腫瘍細胞を死滅させることができる可能性があります。 ユーイング肉腫の治療において、放射線療法および/または手術の有無にかかわらず、併用化学療法がより効果的であるかどうかはまだわかっていません。
目的：この無作為化第III相試験では、ユーイング肉腫患者の治療において、末梢幹細胞移植、放射線療法、手術の有無にかかわらず、さまざまな併用化学療法レジメンがどの程度効果があるかを研究している。

主要な
- ビンクリスチン、ダクチノマイシン、イホスファミド、エトポシド（VIDE）を含む標準導入化学療法とそれに続くビンクリスチン、ダクチノマイシン、イホスファミドを含む地固め化学療法で治療したユーイング家腫瘍患者の無イベント生存期間および全生存期間と、高用量のブスルファンおよびメルファランを比較した。 (Bu-Mel) に続いて、放射線療法および/または手術を伴うまたは伴わない自家末梢血幹細胞 (PBSC) 移植。
二次
- これらの患者における EWS-Flil 転写物の予後的重要性を判断します。
- これらの患者における、EWS-Flil 転写物の有無によって決定される、骨髄および PBSC における微小疾患の頻度と予後値を決定します。
- これらの患者における VIDE 導入化学療法の実現可能性と毒性を判断します。

概要: これは無作為化された多施設研究です。 患者は、年齢および原発腫瘍の局所治療に従って層別化されます（はい vs いいえ）。
患者は、1日目にビンクリスチンIV、3時間かけてイホスファミドIV、4時間かけてドキソルビシンIV、1～3日目に1時間かけてエトポシドIVを含む導入化学療法を受ける（VIDE）。 許容できない毒性がない限り、治療は 21 日ごとに 4 コース繰り返されます。 末梢血幹細胞 (PBSC) は、コース 3 および/または 4 の後に収集されます。患者は、コース 4 の後に評価されます。軸方向の腫瘍により早期放射線治療が必要な患者、または脳および/または脊髄への放射線治療が必要な患者早期放射線療法を必要としない患者はグループ 2 に割り当てられます。
- グループ 1: 患者はさらに 2 コースの VIDE 導入化学療法を受けます (コース 5 および 6)。 軸方向腫瘍への放射線療法を必要とする患者は、週に 5 日の同時放射線療法も受けます。 その後、一部の患者は腫瘍の外科的切除を受ける場合があります。 その後、すべての患者は 1 日目にビンクリスチン IV を受け、1 日目と 2 日目には 3 時間かけてダクチノマイシン IV とイホスファミド IV を受けます (VAI)。 治療は 8 コース (コース 7 ～ 14) の間、21 日ごとに繰り返されます。 脳および/または脊髄への放射線療法を必要とする患者は、同時に放射線療法も受けます。
グループ 2: 患者はさらに 2 コースの VIDE 導入化学療法を受けます (コース 5 および 6)。 その後、一部の患者は腫瘍の外科的切除を受ける場合があります。 すべての患者は、グループ 1 と同様に 1 コースの VAI 化学療法を受けます。 患者は、2つの地固め療法群のうちの1つにランダムに割り当てられます。
- アーム I: 患者はさらに 7 コースの VAI 化学療法を受けます (コース 8 ～ 14)。 切除不能、部分切除、または切除が不十分な疾患を有する患者は、6～12日間の同時全肺放射線療法を受けます。
- アーム II: 患者は、-6 日目から-3 日目まで 6 時間ごとに経口ブスルファンを投与され、-2 日目に 30 分間かけてメルファラン IV を投与される高用量化学療法を受けます。 患者は0日目に自家PBSC IVを受けます。切除不能、部分切除、または切除が不十分な疾患を有する患者は、少なくとも5週間、週5日の同時放射線療法を受けます。
患者は4年間は3か月ごと、1年間は6か月ごと、その後は定期的に追跡されます。

- ユーイング肉腫
- 末梢原始神経外胚葉腫瘍
以下の基準のいずれかを満たす病気:
- 切除可能な限局性疾患（腫瘍体積200mL未満）
- 以前に診断時に切除された局所疾患
- 切除不能な疾患（腫瘍体積が少なくとも200 mL）だが、局所制御としての放射線療法は遅れる可能性がある
- 早期の放射線治療が必要な局所疾患
- 肺および/または胸膜転移のみ
- 肺外/胸膜転移 (骨格、骨髄、リンパ節)
- 確定生検から 45 日以内

実験的：グループ1
患者はさらに 2 コースの VIDE 導入化学療法を受けます (コース 5 および 6)。
軸方向腫瘍への放射線療法を必要とする患者は、週に 5 日の同時放射線療法も受けます。
その後、一部の患者は腫瘍の外科的切除を受ける場合があります。
その後、すべての患者は 1 日目にビンクリスチン IV を受け、1 日目と 2 日目には 3 時間かけてダクチノマイシン IV とイホスファミド IV を受けます (VAI)。
治療は 8 コース (コース 7 ～ 14) の間、21 日ごとに繰り返されます。
脳および/または脊髄への放射線療法を必要とする患者は、同時に放射線療法も受けます。

患者は0日目に自家PBSC IVを受けます。切除不能、部分切除、または切除が不十分な疾患を有する患者は、少なくとも5週間、週5日の同時放射線療法を受けます。
