米国では 1,600 万人以上の成人が慢性閉塞性肺疾患 (COPD) に苦しんでおり、死因の第 4 位となっています。(マクロリー) 他。 1190-209)。 COPD による直接医療費は 180 億ドルに上ります。(マクロリー) 他。 1190-209)。 酸素療法は、安静時に重度の低酸素血症を伴う COPD 患者の死亡率を低下させることが示されています。 酸素療法は現在、耐久性のある医療機器の最大の支出を占めています。 COPD 患者に対する酸素療法の現在の要件には、安静時または運動時の paO2 または spO2 がそれぞれ 55 mmHg または 88% であることが含まれます。 我々は、6 分間の歩行中の継続的な spO2 モニタリングに基づいて、95% 未満の安静時 spO2 が COPD 患者の運動誘発性低酸素血症のリスクを予測できることを示しました (Knower et al. 732-36)。 6 分間の歩行の再現性については、病院での検査、模擬家庭での検査、実際の家庭での検査 (Guyatt) の観点から検討されていますが、その相互変動については十分に研究されていません。
酸素処方のための 6 分間の歩行の信頼性と相互変動性を調査することを提案します。 現在、2 つのプログラムが地域社会の COPD 患者に呼吸リハビリテーションを提供しています。 患者は監督の下で 6 分間の散歩を行い、これらのプログラムの一環として定期的にパルスオキシメトリーによってモニタリングされます。 6 分間の歩行で酸素処方の信頼性を判断するために、酸素濃度測定データを取得する予定です。 6分間の歩行を行っている患者様が登録対象となります。 6 分間の歩行に関するプロトコルはすでに導入されており、両方の肺リハビリテーション プログラムのそれぞれの当局によって承認されています。 これらの患者は、その日の spO2 の信頼性を判断するために、近くで別々に 6 分間の散歩を繰り返します。 統計分析は、3 次元マトリックスでのテスト間一致のカッパ統計によって実行されます。
