상세 설명
미국에서 1,600만 명이 넘는 성인이 만성 폐쇄성 폐질환(COPD)을 앓고 있으며 사망 원인 4위를 차지하고 있습니다.(McCrory 외. 1190-209). COPD는 180억 달러의 직접적인 의료 비용을 차지합니다.(McCrory 외. 1190-209). 산소 요법은 안정 시 중증 저산소혈증이 있는 COPD 환자의 사망률을 감소시키는 것으로 나타났습니다. 산소 요법은 이제 내구성 있는 의료 장비에 대한 가장 큰 지출을 차지합니다. COPD 환자를 위한 산소 요법에 대한 현재 요구 사항에는 각각 55mmHg 또는 88%의 휴식 또는 운동 paO2 또는 spO2가 포함됩니다. 우리는 95% 미만의 휴식기 spO2가 COPD 환자(Knower et al. 732-36)에서 6분 동안 걷는 동안 지속적인 spO2 모니터링을 기반으로 운동으로 인한 저산소혈증의 위험이 있음을 예측합니다. 6분 걷기의 재현성은 병원 테스트, 모의 가정 테스트 및 실제 가정 테스트(Guyatt) 측면에서 다루어졌지만 상호 변동성은 잘 연구되지 않았습니다.
우리는 산소 처방을 위한 6분 걷기의 신뢰도와 상호 변동성을 조사할 것을 제안합니다. 현재 두 개의 프로그램이 지역 사회에서 COPD 환자를 위한 폐 재활을 제공하고 있습니다. 환자는 감독 하에 6분간 걷기를 수행하고 이러한 프로그램의 일환으로 일상적으로 맥박 산소측정기로 모니터링합니다. 6분 걷기를 이용한 산소 처방의 신뢰도를 판단하기 위해 산소 측정 데이터를 캡처할 계획입니다. 6분 걷기를 수행하는 환자는 등록할 수 있습니다. 6분 걷기에 대한 프로토콜은 이미 시행 중이며 폐 재활 프로그램 모두에서 해당 기관의 승인을 받았습니다. 이 환자들은 특정 날짜에 spO2의 신뢰성을 확인하기 위해 6분 동안 가까운 거리에서 반복적으로 걸을 것입니다. 통계 분석은 3차원 매트릭스에서 테스트 간 일치에 대한 kappa 통계에 의해 수행됩니다.
