目的
STADIAパイロット研究は、急性冠症候群(ACS)直後の経皮ニコチン使用に関連する長期的な安全性と有効性の結果を調査する3年間の多施設試験の実施の実現可能性を評価するために使用されます。
根拠
最初に ACS を生き延びた患者は、心臓イベントの再発または心血管死のリスクが高くなります。 最初の 6 か月間だけでも、患者の 12% が死亡するか、別の ACS を発症し、さらに 23% が狭心症のために血行再建術を必要とします。 禁煙に成功した患者は、再発性ACSまたは心血管死を翌年以内に発症する可能性が50%以上低下します。 ACS を生き延びた後、ほとんどの患者は喫煙をやめる意欲が非常に高くなります。 しかし、ニコチンは中毒性の高い向精神薬であるため、禁煙を試みる患者の最大 60% が再発します。 ACS 後の入院は、入院中の喫煙が禁止されているため、喫煙者が禁煙する機会を提供します。 しかし、入院中の禁酒は、強いニコチン離脱症状を引き起こし、これが喫煙再発率の高さの主な原因です。 ニコチン置換療法 (NRT) は、これらの症状の多くを改善し、喫煙者が退院後もタバコの使用を控え続ける可能性を高めます。 利用可能なNRTのうち、経皮ニコチンは、ニコチンガムや吸入器よりもニコチンをよりゆっくりと(より低いピーク値で)送達するため、心血管への影響が最も少ない. 経皮ニコチンは、プラセボと比較して長期禁煙率を2倍以上に高めますが、ACS後のニコチンの心血管への影響の可能性はまだ体系的に評価されていないため、ACSの直後の数週間に医師によって処方されることはめったにありません.
メソッド
合計 30 人の患者が、ユダヤ総合病院 (モントリオール、QC) および協力センターから ACS に罹患してから 48 時間以内に無作為に割り付けられます。 無作為化の前に、すべての患者は標準化された禁煙カウンセリングを受けます。 試験の包含および除外基準を満たす被験者は、ACSに苦しんでから48時間以内に無作為に割り付けられ、研究期間の1日目または2日目に経皮ニコチンパッチを着用します。 被験者は、治療日の朝にパッチを適用し、24 時間そのままにしておきます。 患者は、主治医と禁煙戦略について話し合うようにアドバイスされ、その後のケアはこの医師の裁量に委ねられます。 24時間のホルターモニタリングを使用して、心拍数、不整脈の発生率、および研究期間の1日目と2日目の虚血イベントの持続時間を測定します。 収縮期および拡張期血圧は、24 時間の外来血圧モニタリング (ABPM) によって評価されます。 ABPM は、研究期間の 1 日目と 2 日目に使用されます。 1日目に、研究看護師は、ニコチン離脱症状の重症度、副作用、喫煙の再発、もしあれば、喫煙したタバコの本数、および現在服用している薬を記録します。 7日目と14日目に、患者は電話で連絡を受け、ニコチン離脱症状の重症度、喫煙の再発、もしそうなら、何本のタバコを吸ったか、現在服用している薬を判断します. 心血管イベントは、患者のモニタリングと研究看護師によって評価されます。 Hughes-Hatsukami症状スケールによるニコチン禁断症状と一般的なニコチンパッチの副作用は、5段階のスケールで評価され、1、7、および14日目の研究看護師の問い合わせによって決定されます(6)。 一酸化炭素モニターを使用して、ベースラインでの呼気 CO のレベルを測定します。 期限切れの CO レベルが 10 ppm を超える参加者は、喫煙者として分類されます。
意義
安全で効果的であることが示されれば、経皮ニコチンは、毎年 ACS に苦しむ 20,000 人のカナダ人喫煙者の再発性心疾患の二次予防に大きな影響を与えるでしょう。

- 18歳以上
- 現在の喫煙者、過去 1 年間、平均して 1 日あたり 10 本以上のタバコを吸っている
- -酵素陽性のACS(トロポニンT、トロポニンI、またはCK-MD)に罹患し、48時間以上の入院を計画している
- 禁煙へのモチベーション
除外基準:
- -禁煙のための医学療法の現在の使用
- アルコールまたは規制薬物の乱用歴
- 重度の皮膚炎の病歴
- 投薬を必要とする不安定な精神疾患の現在の診断
- 別の状態による入院の合併症としてACSに苦しんだ(つまり、 術後)
- 妊娠または授乳
- フォローアップに利用できない可能性が高い
- 英語またはフランス語を読んで理解できない
