목적
STADIA 시범 연구는 급성 관상동맥 증후군(ACS) 직후 경피 니코틴 사용과 관련된 장기적인 안전성 및 효능 결과를 조사하는 3년 다기관 시험 수행 가능성을 평가하는 데 사용될 예정입니다.
이론적 해석
초기에 ACS에서 살아남은 환자는 재발성 심장 사건 또는 심혈관 사망의 위험이 높습니다. 처음 6개월 이내에 환자의 12%가 사망하거나 또 다른 ACS를 겪게 되며, 또 다른 23%는 협심증으로 인해 혈관재생술이 필요합니다. 금연할 수 있는 환자는 다음 해에 재발성 ACS 또는 심혈관계 사망을 겪을 가능성을 50% 이상 줄입니다. ACS에서 살아남은 후 대부분의 환자는 금연에 대한 강한 동기를 갖게 됩니다. 그러나 니코틴은 매우 중독성이 강한 향정신성 약물이므로 금연을 시도하는 환자의 최대 60%가 재발합니다. 행동 양식
총 30명의 환자가 유대인 종합병원(QC 몬트리올) 및 협력 센터에서 ACS를 앓은 후 48시간 이내에 무작위로 배정됩니다. 무작위 배정 전에 모든 환자는 표준화된 금연 상담을 받게 됩니다. 시험에 대한 포함 및 제외 기준을 만족하는 피험자는 연구 기간의 1일 또는 2일에 경피 니코틴 패치를 착용하도록 ACS를 앓은 후 48시간 이내에 무작위 배정됩니다. 대상자는 치료 당일 아침에 패치를 적용하고 24시간 동안 유지합니다. 환자는 치료 의사와 금연 전략에 대해 상의하도록 조언을 받을 것이며 후속 치료는 이 의사의 재량에 맡길 것입니다. 24시간 Holter 모니터링을 사용하여 연구 기간의 1일과 2일에 심박수, 부정맥 발생률 및 허혈성 사건의 지속 시간을 측정합니다. 수축기 및 이완기 혈압은 24시간 보행 혈압 모니터링(ABPM)을 통해 평가됩니다. ABPM은 연구 기간의 1일과 2일에 사용됩니다. 1일차에 연구 간호사는 니코틴 금단 증상의 중증도, 부작용, 흡연 재발, 그렇다면 흡연한 담배 수, 현재 복용 중인 약물을 기록합니다. 7일과 14일에 환자는 니코틴 금단 증상의 중증도, 흡연 재발 여부, 흡연한 경우 얼마나 많은 담배를 피웠는지, 현재 복용 중인 약물을 확인하기 위해 전화로 연락을 받게 됩니다. 심혈관 사건은 환자 모니터링 및 연구 간호사에 의해 평가될 것입니다. Hughes-Hatsukami 증상 척도에 따른 니코틴 금단 증상과 5점 척도로 중증도를 평가한 일반 니코틴 패치 부작용은 1일, 7일 및 14일(6)에 연구 간호사 문의를 통해 결정됩니다. 일산화탄소 모니터는 기준선에서 만료된 CO 수준을 측정하는 데 사용됩니다. 만료된 CO 수준이 10ppm을 초과하는 참가자는 흡연자로 분류됩니다.

안전하고 효과적인 것으로 밝혀지면 경피 니코틴은 매년 ACS로 고통받는 20,000명의 캐나다 흡연자에게 재발성 심장 사건의 2차 예방에 큰 영향을 미칠 것입니다.
