この研究では、都市近郊のヒト免疫不全ウイルス(HIV)に感染した成人および小児患者を対象とした第一選択の結核(TB)診断検査として、Xpert Mycobacterium Tuberculosis/Rifampicin(MTB/RIF)を評価し、正確な症例への影響を判定します。これらの環境での検出と治療の開始。 さらに、Determine TB-Lipoarabinomannan (LAM) Ag® は、ザンビアの HIV 感染患者に対する結核診断アルゴリズム内で、単独または Xpert MTB/RIF と組み合わせて使用した場合の付加的な診断価値をモデル化するために使用されます。
準実験的な「ビフォー・アフター」研究デザインが、都市近郊の同様の 2 つの病院で使用されます。 各施設はまず標準治療(SOC)段階を実施する。この段階では、ザンビアの HIV 治療クリニックの現在の標準治療に従って、HIV に感染した成人および小児の結核疑い者の将来コホートに対して結核のスクリーニングが行われる。 その後、SOC フェーズの最後の 1 か月間で募集されたすべての患者の診断精密検査を完了できるように、4 週間の「介入ウォッシュアウト」期間が続きます。
第 2 フェーズ (Xpert MTB/RIF フェーズ) は、「介入ウォッシュアウト」期間の直後に開始されます。 この段階では、HIV に感染した成人および小児の結核容疑者の第 2 コホートが、Xpert MTB/RIF アルゴリズムを使用して結核の検査を受けます。 各フェーズは約 6 か月、または成人の目標サンプルサイズに達するまで続きます。 結核の診断を確認し、適切な治療が行われたかどうかを判断するために、研究中にマイコバクテリアの培養が行われます。 「適切な治療」とは、培養陽性患者のスクリーニング開始、または培養陰性患者の結核ではないという正確な診断から 4 週間以内に患者が ATT を開始されたことを意味します。 培養結果は、結果が入手可能になり次第、患者ケアのために CIDRZ Lab からリリースされます。
どちらの研究段階でも、参加者は Determine TB-LAM Ag® アッセイによる検査のために尿サンプルを提出するように求められます。 尿検体は研究施設で収集され、すべての手順はルサカのCIDRZ中央研究所で標準的な実験プロトコルと品質保証手順に従って実行されます。 Determine TB-LAM Ag® はまだ世界保健機関 (WHO) によって承認されておらず、ザンビア保健省からも結核診断について承認されていないため、Determine TB-LAM Ag® アッセイの結果は患者ケアには使用されません。

包含基準:
- 15歳以上(「大人」)、または15歳未満(「子供」)。
- HIV に感染し、チョンウェ地区病院とカフエ地区病院で HIV ケアに登録されている
- 少なくとも6か月間は郡病院での治療を継続するつもりです。
- ザンビア国家ガイドラインによる結核の疑い [31] 。
- 署名済みのインフォームド・コンセント(参加者が 18 歳未満の場合は保護者の同意)を提供する意思がある。
- 位置情報を提供し、電話または自宅訪問による連絡を許可する意思がある (または親または保護者が意思を持つ)
除外基準:
- 過去6か月以内に結核と診断された、または過去3か月以内に結核治療を受けた
- 研究の目的を妨げる可能性のある別の研究に登録されている(例: TB-HAART)
