閉塞性睡眠時無呼吸 (OSA) は、一般的な睡眠関連の呼吸障害です。 CPAP は、生活の質を改善し、心血管イベントの発生率を大幅に低下させる OSA 患者の第一選択治療として確立されています。 ただし、周術期における CPAP の有効性に関するエビデンスはほとんどありません。 Brar らによって実施された前向きコホート研究。は、処方された CPAP 療法の使用にもかかわらず、OSA 患者の 18% が手術後の夜に低酸素状態になったことを発見し、CPAP 治療を受けた OSA 手術患者の術後オキシメトリーモニタリングを提唱しました。
研究者は、OSA の外科手術患者における術後の低酸素症およびその他の合併症の軽減における CPAP コンプライアンスの効果を判断したいと考えています。 この研究の結果は、OSA と診断されて CPAP を処方された手術歴のある外科患者における CPAP の非遵守率、および術後転帰、特に酸素飽和度に対する CPAP の非遵守の影響に関する貴重なデータを提供します。
この研究では、CPAP処方の有無にかかわらず、OSAと診断されたすべての外科患者に、研究に登録するためのインフォームドコンセントを求めます。 すべての研究参加者は、自宅で腕時計型パルスオキシメータを使用して、手術前の 1 晩と手術後の最初の 3 晩、病院で酸素飽和度の低下を監視されます。
登録された被験者は、自分のCPAPユニット(病院の方針に従って病院の工学部門によって認定されたもの)とマスクを病院に持参します。 CPAP 処方箋のない OSA 患者は周術期にフォローアップされ、OSA の重症度を判断し、CPAP 処方箋がない理由を確認します。
術前および術後の CPAP 使用データは、患者へのインタビュー、カルテのレビュー、およびスマート CPAP デバイスでの記録の監視を通じて収集されます。

除外基準:
- 彼らは、研究にインフォームドコンセントを与えることを望まないか、または与えることができません。
