A apneia obstrutiva do sono (OSA) é um distúrbio respiratório comum relacionado ao sono. O CPAP é estabelecido como o tratamento de primeira linha para pacientes com AOS, com melhora na qualidade de vida e redução significativa na incidência de eventos cardiovasculares. No entanto, faltam evidências sobre a eficácia do CPAP no período perioperatório. Um estudo de coorte prospectivo conduzido por Brar et al. descobriram que, apesar do uso de terapia CPAP prescrita, 18% dos pacientes com AOS tornaram-se hipóxicos na noite seguinte à cirurgia e defenderam o monitoramento da oximetria pós-operatória em pacientes cirúrgicos com AOS tratados com CPAP.
Os investigadores gostariam de determinar o efeito da adesão ao CPAP na redução da hipóxia pós-operatória e outras complicações em pacientes cirúrgicos com AOS. Os resultados deste estudo forneceriam dados valiosos sobre a taxa de não adesão ao CPAP em pacientes cirúrgicos com história de AOS diagnosticada e prescrição de CPAP e o impacto da não adesão ao CPAP nos resultados pós-operatórios, especialmente na saturação de oxigênio.
Neste estudo, todos os pacientes cirúrgicos diagnosticados com AOS com ou sem prescrição de CPAP serão abordados para consentimento informado para inclusão no estudo. Todos os participantes do estudo serão monitorados quanto à dessaturação de O2 usando o oxímetro de pulso em casa por uma noite no pré-operatório e nas primeiras 3 noites no pós-operatório no hospital.
Os participantes inscritos trarão suas próprias unidades CPAP (certificadas pelo departamento de engenharia do hospital de acordo com a política do hospital) e máscara para o hospital. Os pacientes com AOS sem prescrição de CPAP serão acompanhados no período perioperatório para determinar a gravidade de sua OSA e para verificar a justificativa de não prescrição de CPAP.
Os dados de uso de CPAP pré e pós-operatório serão coletados por meio de entrevistas com o paciente, revisão de prontuário e registro de monitoramento no dispositivo CPAP inteligente.

Critério de inclusão:
- Todos os pacientes cirúrgicos adultos (>18 anos) com diagnóstico de AOS com ou sem prescrição de CPAP.
- Programado para uma cirurgia não cardíaca (cirurgia geral, ortopedia, urologia, cirurgia plástica e coluna) que se prevê que necessite de internamento pós-operatório superior a uma noite.
Critério de exclusão:
- Eles não querem ou não podem dar consentimento informado para o estudo.
- Eles estão recebendo oxigênio suplementar no pré-operatório (diurno ou noturno).
- Elas estão grávidas.
- A cirurgia inclui amigdalectomia, septoplastia, uvuloplastia, faringoplastia, traqueostomia ou ventilação mecânica pós-operatória prolongada (> 48 horas) dos pulmões é antecipada. É provável que esses procedimentos/intervenções curem ou pelo menos modifiquem a gravidade da AOS.
