La apnea obstructiva del sueño (AOS) es un trastorno respiratorio común relacionado con el sueño. CPAP se establece como el tratamiento de primera línea para pacientes con AOS con una mejora en la calidad de vida y una reducción significativa en la incidencia de eventos cardiovasculares. Sin embargo, faltan pruebas sobre la eficacia de la CPAP en el período perioperatorio. Un estudio de cohorte prospectivo realizado por Brar et al. encontraron que a pesar del uso de la terapia CPAP prescrita, el 18% de los pacientes con AOS se volvieron hipóxicos la noche posterior a la cirugía y recomendaron el monitoreo de oximetría posoperatoria en pacientes quirúrgicos con AOS tratados con CPAP.
A los investigadores les gustaría determinar el efecto del cumplimiento de la CPAP en la reducción de la hipoxia posoperatoria y otras complicaciones en los pacientes quirúrgicos con OSA. Los resultados de este estudio proporcionarían datos valiosos sobre la tasa de incumplimiento de CPAP en los pacientes quirúrgicos con antecedentes de AOS diagnosticada y prescripción de CPAP y el impacto del incumplimiento de CPAP en los resultados postoperatorios, especialmente la saturación de oxígeno.
En este estudio, todos los pacientes quirúrgicos con diagnóstico de AOS con o sin prescripción de CPAP recibirán el consentimiento informado para inscribirse en el estudio. Se controlará la desaturación de O2 de todos los participantes del estudio mediante el uso de un oxímetro de pulso de reloj de pulsera en el hogar durante una noche antes de la operación y las primeras 3 noches después de la operación en el hospital.
Los sujetos inscritos traerán sus propias unidades de CPAP (certificadas por el departamento de ingeniería del hospital según la política del hospital) y máscara al hospital. Los pacientes con AOS sin prescripción de CPAP serán objeto de un seguimiento perioperatorio para determinar la gravedad de su AOS y determinar la justificación de la no prescripción de CPAP.
Los datos de uso de CPAP antes y después de la operación se recopilarán entrevistando al paciente, revisando el historial y monitoreando el registro en el dispositivo inteligente de CPAP.

Criterios de inclusión:
- Todos los pacientes quirúrgicos adultos (>18 años) con diagnóstico de AOS con o sin prescripción de CPAP.
- Programado para una cirugía no cardíaca (cirugía general, ortopedia, urología, cirugía plástica y columna) que se espera que requiera una hospitalización postoperatoria de más de una noche.
Criterio de exclusión:
- No quieren o no pueden dar su consentimiento informado para el estudio.
- Reciben oxígeno suplementario antes de la operación (durante el día o la noche).
- están embarazadas.
- Se prevé cirugía incluida amigdalectomía, septoplastia, uvuloplastia, faringoplastia, traqueotomía o ventilación mecánica postoperatoria prolongada (> 48 horas) de los pulmones. Es probable que estos procedimientos/intervenciones curen o al menos modifiquen la gravedad de la AOS.
