L'apnée obstructive du sommeil (AOS) est un trouble respiratoire courant lié au sommeil. La CPAP est établie pour être le traitement de première intention des patients atteints d'AOS avec une amélioration de la qualité de vie et une réduction significative de l'incidence des événements cardiovasculaires. Cependant, les preuves de l'efficacité de la CPAP dans la période périopératoire font largement défaut. Une étude de cohorte prospective menée par Brar et al. ont constaté que malgré l'utilisation de la thérapie CPAP prescrite, 18% des patients atteints d'AOS sont devenus hypoxiques la nuit suivant la chirurgie et ils ont préconisé une surveillance oxymétrique postopératoire chez les patients chirurgicaux AOS traités par CPAP.
Les chercheurs aimeraient déterminer l'effet de l'observance du CPAP dans la réduction de l'hypoxie postopératoire et d'autres complications chez les patients chirurgicaux atteints d'AOS. Les résultats de cette étude fourniraient des données précieuses sur le taux de non-conformité à la CPAP chez les patients chirurgicaux ayant des antécédents d'OSA diagnostiqué et une prescription de CPAP et l'impact de la non-conformité à la CPAP sur les résultats postopératoires, en particulier la saturation en oxygène.
Dans cette étude, tous les patients chirurgicaux OSA diagnostiqués avec ou sans ordonnance CPAP seront approchés pour obtenir un consentement éclairé pour s'inscrire à l'étude. Tous les participants à l'étude seront surveillés pour la désaturation en O2 à l'aide d'un oxymètre de pouls à montre-bracelet à domicile pendant une nuit avant l'opération et les 3 premières nuits après l'opération à l'hôpital.
Les sujets inscrits apporteront leurs propres unités CPAP (certifiées par le service d'ingénierie hospitalière conformément à la politique de l'hôpital) et un masque à l'hôpital. Les patients atteints d'AOS sans prescription de CPAP seront suivis en périopératoire pour déterminer la gravité de leur AOS et pour déterminer la raison de l'absence de prescription de CPAP.
Les données d'utilisation pré et post-opératoires du CPAP seront collectées en interrogeant le patient, en examinant le dossier et en surveillant l'enregistrement dans l'appareil CPAP intelligent.

Critère d'intégration:
- Tous les patients chirurgicaux adultes (>18 ans) avec un diagnostic d'AOS avec ou sans ordonnance de CPAP.
- Prévu pour une chirurgie non cardiaque (chirurgie générale, orthopédie, urologie, chirurgie plastique et rachis) qui devrait nécessiter une hospitalisation postopératoire de plus d'une nuit.
Critère d'exclusion:
- Ils ne veulent pas ou ne peuvent pas donner leur consentement éclairé à l'étude.
- Ils sont sous oxygène d'appoint en préopératoire (jour ou nuit).
- Elles sont enceintes.
- Une intervention chirurgicale comprenant une amygdalectomie, une septoplastie, une uvuloplastie, une pharyngoplastie, une trachéotomie ou une ventilation mécanique postopératoire prolongée (> 48 heures) des poumons est prévue. Ces procédures/interventions sont susceptibles de guérir ou au moins de modifier la sévérité du SAOS.
