説明
包含基準:
- -登録から5年以内の小細胞肺がん以外の非血液悪性腫瘍の病理学的に証明された診断。
- -測定可能な脳転移を有し、ガドリニウム造影MRIを登録前30日以内に取得した患者。 MRIを使用しないCTベースの画像診断を受けた患者は、この試験に登録できません。
- -造影剤を使用した脳のMRIでサイズが3cm以上6cm以下と定義された標的病変の外科的切除なし
- -以前の全脳放射線療法(WBRT)は許可されています
- 18歳以上
- -カルノフスキーパフォーマンスステータス≥60またはECOG≤2
- 出産の可能性のある女性と男性の参加者は、適切な避妊を実践する必要があります
- 登録前30日以内の病歴/健康診断
- 余命>3ヶ月
- -以前にSRSで他の病変を治療した場合、患者は登録が許可されます
- FSRT で治療される主要な病変が 1 つある限り、複数の病変を持つ患者は許可されます。 他の病変は、治療する医師の裁量でSRSまたはFSRTと同時に治療される場合がありますが、研究のエンドポイントには寄与しません。
除外基準:
- -脳脊髄液検査(CSF)に基づいて、決定的な軟髄膜転移を有する患者
- 治療中の化学療法または標的薬剤の計画。 すべてのホルモン療法と骨支持療法は、患者が研究している間継続することができます。
- -埋め込み金属デバイスなどの強化された MRI イメージングの禁忌。 ただし、MRI対応の埋め込み型デバイスを使用している患者は許可されます。
- 小細胞肺癌および生殖細胞悪性腫瘍に起因する測定可能な脳転移を有する患者
- うっ血性心不全、不安定狭心症、心不整脈、および制御不能な発作活動などの制御不能な併発疾患。
- -放射線療法による標的病変の以前の治療。

実験的:分別された立体放射線療法(FSRT)
この研究は、継続的な再評価方法(CRM)を採用して最大耐量を決定するという点でユニークです。
登録された後続の各患者の適切な用量レベルの情報は、試験に登録された以前の患者のDLTに基づいて決定されます。

初期開始用量は7 Gy x5 = 35 Gyになり、計画ターゲットボリューム(PTV)に配信されます。
放射線療法は、1.5〜2週間以内に定位体放射療法(SBRT)スケジュールが完了するように、1日おきに配信されます。

Maximum Tolerated Dose (MTD)
時間枠:Within 1 year of initiation of treatment
|
Determine the maximum tolerated dose (MTD) associated with a 20% probability of dose limiting toxicity (DLT).
DLT is defined as grade 3 or higher toxicities (per CTCAE version 4.03; MedDRA version 12.0) that occur within 1 year of treatment start and are either possibly, probably, or definitively related to radiotherapy (RT), and related to the specific symptoms.

Response to Treatment
時間枠:Up to 1 year following initiation of treatment
|
Response to treatment will be assessed based on the percentage of patients who achieve either a complete response (CR), partial response (PR), or stable disease (SD), or progression (P) in response to FSRT treatment. Best response will be summarized. Response criteria will be assessed as follows: Complete Response (CR): Defined as follow up imaging which shows that complete radiographic disappearance of brain metastasis Partial Response (PR): Decrease of >50% in the size of each lesion radiographically, using perpendicular diameters Stable Disease (SD): 0 to 50% reduction in size of each lesion radiographically, using perpendicular diameters Progression (P): Defined as > 25% increase in the size of each lesion or a new, non-contiguous lesion developed outside of the radiosurgical volume |
