Descripción
Criterios de inclusión:
- Diagnóstico patológicamente probado de una neoplasia maligna no hematológica distinta del cáncer de pulmón de células pequeñas dentro de los 5 años posteriores al registro.
- Pacientes con metástasis cerebrales medibles que tienen una resonancia magnética realzada con gadolinio obtenida dentro de los 30 días anteriores al registro. Los pacientes con imágenes de diagnóstico basadas en TC sin resonancia magnética no podrán inscribirse en este ensayo.
- Sin resección quirúrgica de la lesión diana, definida como ≥3 cm pero ≤6 cm de tamaño en la resonancia magnética del cerebro con contraste
- Se permite la terapia previa de radiación cerebral total (WBRT)
- Edad ≥ 18 años
- Estado funcional de Karnofsky ≥ 60 o ECOG ≤2
- Las mujeres en edad fértil y los participantes masculinos deben practicar métodos anticonceptivos adecuados.
- Historial/examen físico dentro de los 30 días anteriores al registro
- Esperanza de vida >3 meses
- Los pacientes pueden inscribirse si han sido tratados previamente de otras lesiones con SRS
- Se permiten pacientes con lesiones múltiples, siempre que haya una lesión dominante que se tratará con FSRT. Otras lesiones pueden tratarse simultáneamente con SRS o FSRT a discreción del médico tratante, pero no contribuirán a los criterios de valoración del estudio.
Criterio de exclusión:
- Pacientes con metástasis leptomeníngeas definitivas, según el examen del líquido cefalorraquídeo (LCR)
- Plan de quimioterapia o agentes dirigidos durante el tratamiento. Toda la terapia hormonal y la terapia de apoyo óseo se pueden continuar mientras el paciente está en estudio.
- Contraindicación para imágenes de resonancia magnética mejorada, como dispositivos metálicos implantados. Sin embargo, se permiten pacientes con dispositivos implantados compatibles con MRI.
- Pacientes con metástasis cerebrales medibles resultantes de cáncer de pulmón de células pequeñas y malignidad de células germinales
- Enfermedad intercurrente no controlada, como insuficiencia cardíaca congestiva, angina inestable, arritmia cardíaca y actividad convulsiva no controlada.
- Tratamiento previo de las lesiones diana con radioterapia.

Experimental: Radioterapia estereotáctica fraccionada (FSRT)
Este estudio es único porque emplea una metodología de reevaluación continua (CRM) para determinar la dosis máxima tolerada.
La información para el nivel de dosis adecuado para cada paciente posterior inscrito se determinará en base a DLT de pacientes anteriores inscritos en el ensayo.

La dosis inicial inicial será de 7 Gy x5 = 35 Gy entregada al volumen objetivo de planificación (PTV).
La radioterapia se administrará cada dos días para que el programa de radioterapia del cuerpo estereotáctico (SBRT) se complete dentro de 1.5-2 semanas.

Maximum Tolerated Dose (MTD)

|
Determine the maximum tolerated dose (MTD) associated with a 20% probability of dose limiting toxicity (DLT).
DLT is defined as grade 3 or higher toxicities (per CTCAE version 4.03; MedDRA version 12.0) that occur within 1 year of treatment start and are either possibly, probably, or definitively related to radiotherapy (RT), and related to the specific symptoms.

Response to Treatment
Periodo de tiempo: Up to 1 year following initiation of treatment
|
Response to treatment will be assessed based on the percentage of patients who achieve either a complete response (CR), partial response (PR), or stable disease (SD), or progression (P) in response to FSRT treatment. Best response will be summarized. Response criteria will be assessed as follows: Complete Response (CR): Defined as follow up imaging which shows that complete radiographic disappearance of brain metastasis Partial Response (PR): Decrease of >50% in the size of each lesion radiographically, using perpendicular diameters Stable Disease (SD): 0 to 50% reduction in size of each lesion radiographically, using perpendicular diameters Progression (P): Defined as > 25% increase in the size of each lesion or a new, non-contiguous lesion developed outside of the radiosurgical volume |
