Beschrijving
Inclusiecriteria:
- Pathologisch bewezen diagnose van een niet-hematologische maligniteit anders dan kleincellige longkanker binnen 5 jaar na registratie.
- Patiënten met meetbare hersenmetastasen die binnen 30 dagen voorafgaand aan de registratie een gadolinium-contrastversterkte MRI hebben gekregen. Patiënten met CT-gebaseerde diagnostische beeldvorming zonder MRI mogen niet deelnemen aan deze studie.
- Geen chirurgische resectie van de doellaesie, gedefinieerd als ≥3 cm maar ≤6 cm groot op MRI van de hersenen met contrast
- Voorafgaande Whole Brain Radiation Therapy (WBRT) is toegestaan
- Leeftijd ≥ 18 jaar
- Karnofsky prestatiestatus ≥ 60 of ECOG ≤2
- Vrouwen die zwanger kunnen worden en mannelijke deelnemers moeten adequate anticonceptie toepassen
- Geschiedenis / Lichamelijk onderzoek binnen 30 dagen voorafgaand aan registratie
- Levensverwachting>3 maanden
- Patiënten mogen zich inschrijven als ze eerder zijn behandeld voor andere laesies met SRS
- Patiënten met meerdere laesies zijn toegestaan, zolang er maar één dominante laesie is die met FSRT wordt behandeld. Andere laesies kunnen gelijktijdig met SRS of FSRT worden behandeld naar goeddunken van de behandelend arts, maar zullen niet bijdragen aan de eindpunten van het onderzoek.
Uitsluitingscriteria:
- Patiënten met definitieve leptomeningeale metastasen, op basis van cerebrospinaal vochtonderzoek (CSF)
- Plan voor chemotherapie of gerichte middelen tijdens de behandeling. Alle hormonale therapieën en botondersteunende therapieën kunnen worden voortgezet terwijl de patiënt in studie is.
- Contra-indicatie voor verbeterde MRI-beeldvorming zoals geïmplanteerde metalen apparaten. Patiënten met geïmplanteerde apparaten die MRI-compatibel zijn, zijn echter wel toegestaan.
- Patiënten met meetbare hersenmetastasen als gevolg van kleincellige longkanker en kiemcelmaligniteit
- Ongecontroleerde bijkomende ziekte zoals congestief hartfalen, onstabiele angina, hartritmestoornissen en ongecontroleerde toevallen.
- Eerdere behandeling van de doellaesies met radiotherapie.

Experimenteel: Gefractioneerde stereotactische radiotherapie (FSRT)
Deze studie is uniek omdat het een continue herbeoordelingsmethode (CRM) gebruikt om de maximaal getolereerde dosis te bepalen.
Informatie voor het juiste dosisniveau voor elke volgende ingeschreven patiënt wordt bepaald op basis van DLT's van eerdere patiënten die zijn ingeschreven in de studie.

De eerste startdosis is 7 Gy X5 = 35 Gy geleverd aan het Planning Target Volume (PTV).
Radiotherapie zal om de andere dag worden afgeleverd, zodat het schema van de stereotactische body radiotatietherapie (SBRT) binnen 1,5-2 weken wordt voltooid.

Determine the maximum tolerated dose (MTD) associated with a 20% probability of dose limiting toxicity (DLT).
DLT is defined as grade 3 or higher toxicities (per CTCAE version 4.03; MedDRA version 12.0) that occur within 1 year of treatment start and are either possibly, probably, or definitively related to radiotherapy (RT), and related to the specific symptoms.

Response to Treatment

|
Response to treatment will be assessed based on the percentage of patients who achieve either a complete response (CR), partial response (PR), or stable disease (SD), or progression (P) in response to FSRT treatment. Best response will be summarized. Response criteria will be assessed as follows: Complete Response (CR): Defined as follow up imaging which shows that complete radiographic disappearance of brain metastasis Partial Response (PR): Decrease of >50% in the size of each lesion radiographically, using perpendicular diameters Stable Disease (SD): 0 to 50% reduction in size of each lesion radiographically, using perpendicular diameters Progression (P): Defined as > 25% increase in the size of each lesion or a new, non-contiguous lesion developed outside of the radiosurgical volume |
