集中治療室 (ICU) は、愛する人が死にかけているのを目の当たりにする家族にとって、悲しみに満ちたストレスの多い場所です。 彼らの苦痛を悪化させ、家族はしばしば患者の「代理」の立場に追い込まれ、患者に代わって生死に関わる決定を下す必要があります。 研究者は、終末期 (EoL) の意思決定が悲嘆によって損なわれることを示しています。悲嘆は、患者の差し迫った死の受け入れを妨げ、患者のケアの質と死に悪影響を及ぼすケアの選択につながります.1-3 これらの状況は、代理母が長引く悲嘆障害 (PGD)、心的外傷後ストレス障害 (PTSD)、および患者が受けた終末期ケアについての決定的後悔の基準を満たすリスクを高めます。 ICU 代理母の 60% 近くが、中度から極度の悲嘆を報告しています。 34% が極度の心的外傷ストレス症状を報告しています.1

- 入院中に ICU に入院し、EoL に近づいている患者。修正された「サプライズ質問」で示されるように、医師は退院まで生き残ることができない患者を特定するよう求められます。
- 代理意思決定者は 18 歳以上である
- 代理の意思決定者は、医師または高度な診療提供者です (つまり、 医師助手、ナース・プラクティショナー) は、指定された医療代理人または意思決定を行う患者代理人として、または患者のカルテまたは代理人の自己申告によってそのように記載されている人を示します
- 代理意思決定者は英語を話す必要があります
