INTREPiD は、高感度迅速診断検査 (HS-RDT) を使用した熱帯熱マラリア原虫の第 1 トリメスター スクリーニング戦略の 2 アーム、非盲検、並行割り当て無作為化試験です。 参加者は、ケニアとコンゴ民主共和国の熱帯熱マラリア原虫の風土病地域で、第 1 トリメスターに出産前クリニックを受診するすべての妊娠の女性です。
同意と登録に続いて、女性は通常の出産前ケアまたは介入に 1:1 で割り当てられます。 介入は、HS-RDTを使用した母体の末梢血における熱帯熱マラリア原虫感染の第1トリメスターでの1回のスクリーニングになります。 HS-RDT 検査で熱帯熱マラリア原虫陽性の女性は、1 コースの Artemether-Lumefantrine (AL) で治療され、その後、通常の出産前ケアに戻されます。
参加者は出産まで追跡され、その後、子孫の生後1か月まで追跡されます。
仮説は、通常の出産前ケアと比較して、第 1 トリメスターの女性を熱帯熱マラリア原虫についてスクリーニングし、AL 陽性の場合に治療することで、妊娠の有害転帰のリスクを軽減するというものです。

- -超音波による妊娠期間が13 6/7週(包括的)未満であると推定される実行可能な単胎妊娠
- HIV未感染
- 学習スケジュールに参加したい
- -妊娠中および出産後1か月間、研究地域に留まる予定
- -研究関連の医療施設で喜んで出産する
除外基準:
- 地域のガイドラインによる専門的なケアへの紹介が必要な高リスク妊娠
- より高度なケアを必要とするスクリーニング時のアクティブな医学的問題
- -スクリーニングの2週間前の抗マラリア薬の領収書
- ArtemetherまたはLumefantrineに対する過去のアレルギー、またはいずれかの薬物の受領を禁止する別の状態
