INTREPiD est un essai randomisé à deux bras, ouvert et à répartition parallèle d'une stratégie de dépistage du 1er trimestre des parasites P. falciparum avec un test de diagnostic rapide à haute sensibilité (HS-RDT). Les participantes seront des femmes de toutes les gravidités se présentant aux cliniques prénatales au cours du 1er trimestre dans des sites endémiques pour le paludisme à P. falciparum au Kenya et en République démocratique du Congo.
Après consentement et inscription, les femmes seront affectées 1: 1 soit aux soins prénatals habituels, soit à l'intervention. L'intervention consistera en un dépistage unique au 1er trimestre de l'infection à P. falciparum dans le sang périphérique maternel avec un HS-RDT. Les femmes dont le test de dépistage de P. falciparum est positif lors du test HS-RDT seront traitées avec une seule cure d'artéméther-luméfantrine (AL), puis renvoyées aux soins prénatals habituels.
Les participants seront suivis jusqu'à l'accouchement, puis pendant le premier mois de vie de leur progéniture.
L'hypothèse est que, par rapport aux soins prénatals habituels, le dépistage des femmes au 1er trimestre pour P. falciparum et leur traitement s'ils sont positifs avec AL réduiront le risque d'issue défavorable de la grossesse.

Critère d'intégration:
- Entre 16 ans et 40 ans (inclus)
- Grossesse unique viable avec un âge gestationnel estimé inférieur à 13 semaines 6/7 (incluses) par échographie
- non infecté par le VIH
- Volonté de participer au programme d'études
- Prévoir de rester dans la zone d'étude pendant toute la durée de la grossesse et 1 mois après l'accouchement
- Disposé à accoucher dans un établissement de santé affilié à l'étude
Critère d'exclusion:
- Grossesse à haut risque nécessitant une orientation vers des soins spécialisés selon les directives locales
- Problème médical actif au moment du dépistage nécessitant des soins de niveau supérieur
- Réception antipaludique dans les 2 semaines précédant le dépistage
- Allergie passée à l'artéméther ou à la luméfantrine ou à une autre condition qui interdit la réception de l'un ou l'autre médicament
- Participation actuelle à une autre étude de recherche clinique
