INTREPiD è uno studio randomizzato a due bracci, in aperto, con assegnazione parallela di una strategia di screening del 1° trimestre per i parassiti di P. falciparum con un test diagnostico rapido ad alta sensibilità (HS-RDT). I partecipanti saranno donne di tutte le gravidanze che si presenteranno alle cliniche prenatali nel 1° trimestre in siti endemici per la malaria da P. falciparum in Kenya e nella Repubblica Democratica del Congo.
Dopo il consenso e l'arruolamento, le donne saranno assegnate 1:1 alle consuete cure prenatali o all'intervento. L'intervento consisterà in un singolo screening nel 1° trimestre per l'infezione da P. falciparum nel sangue periferico materno con HS-RDT. Le donne che risultano positive al test per P. falciparum sui test HS-RDT verranno trattate con un singolo ciclo di artemetere-lumefantrina (AL) e quindi riportate alle consuete cure prenatali.
I partecipanti saranno seguiti durante il parto e poi durante il primo mese di vita della loro prole.
L'ipotesi è che, rispetto alle consuete cure prenatali, lo screening delle donne nel 1° trimestre per P. falciparum e il loro trattamento se positive con AL ridurrà il rischio di un esito avverso della gravidanza.

Età idonea allo studio

Descrizione
Criterio di inclusione:
- Età compresa tra i 16 e i 40 anni (inclusi)
- Gravidanza singola vitale con età gestazionale stimata inferiore a 13 6/7 settimane (incluse) mediante ecografia
- non infetto da HIV
- Disposto a partecipare al programma di studio
- Pianificazione di rimanere nell'area di studio per tutta la durata della gravidanza e 1 mese dopo il parto
- Disposto a consegnare in una struttura sanitaria affiliata allo studio
Criteri di esclusione:
- Gravidanza ad alto rischio che richiede il rinvio a cure specialistiche da parte delle linee guida locali
- Problema medico attivo al momento dello screening che richiede cure di livello superiore
- Ricevimento antimalarico nelle 2 settimane precedenti lo screening
- Pregressa allergia ad Artemether o Lumefantrine o altra condizione che proibisce l'assunzione di entrambi i farmaci
- Attuale partecipazione a un altro studio di ricerca clinica
