具体的な研究目的は次のとおりです。
3 段階のプロセスを通じて、薬物検査技術を地域社会に導入する準備をします。
- ボルチモア警察の犯罪研究所から入手したブルカー アルファ (N=335) と SPARC のアウトリーチ セッションを通じて入手したサンプル (N=100) をテストすることで、薬物検査技術を調整する。
- ボルチモアに本拠を置くハームリダクション団体とのパートナーシップを通じて入手したサンプルをテストし、その結果をゴールドスタンダード(臨床検査)と比較することにより、公衆衛生に関連する違法物質を検出する感度と特異性を測定する薬物検査技術を検証する。
- 薬物使用者 (PWUD) を対象とした Bruker Alpha Fourier Transform Infrared Spectroscopy (FT-IR) 薬物検査機の研究を試験的に実施するため (N=20)。
- PrEPケアの継続へのPWUDの関与を高め、過剰摂取の負担を軽減することを目的とした、地域レベルでモバイルの統合薬物チェックとPrEP介入を開発する。
- 薬物使用者の縦断的コホート(N=600)を募集し、6か月間隔で18か月追跡調査する。
介入を実施し、PWUD の PrEP ケア (主要結果) と過剰摂取予防への取り組みへの影響を評価する。
- PWUD で回避された HIV 症例の観点から、介入の段階的な費用対効果を判断する。
当初の非ヒト対象被験者の研究計画は、ボルチモア警察法医学研究所から調達したサンプルをウィリアム・クラーク博士の研究室で検査したものと、週に3~4回行われるアウトリーチ中にSPARCスタッフと交流した個人から採取したサンプルの間で、Aimの1台の薬物検査機のキャリブレーションに焦点を当てた。 これらの活動は、この研究計画全体を通じて「フェーズ 1」と呼ばれます。 新しいベストプラクティスに基づいて、また全国の他の薬物検査イニシアチブと協議して、研究者らは現在、研究の目的 1 を拡張し、現場での配備に向けて機械を準備するために必要な 3 つのステップを含めました。 最初の (1a) は機械の校正でした。上述したように、地元の医薬品供給に。 2 番目の (1b) は、研究者がこの修正で追加しているもので、機械の機能の追加の検証とゴールドスタンダード技術との比較です。 3 番目 (1c) は、都市全体に展開する前に 20 PWUD でマシンを試験運用しています。

- HIV陰性であると自己報告する
過去 6 か月間における注射薬の使用を報告する、または過去 6 か月間における性的 HIV リスク曝露に関する以下の指標の少なくとも 1 つを報告する:
- 過去6か月以内に性感染症(STI)と診断されたことがある
- 過去6か月以内に「コンドームを使用しない」膣またはアナルセックス
- 尿中薬物物質のスクリーニングのために尿サンプルを提供する意思がある

- 性感染症検査を受けたい
除外基準:
- 高すぎる、飲酒している、または認知障害があると判断された場合、18 歳未満

研究者は、内部で開発されたアンケートを使用して参加者の自己申告を評価することにより、現在の PrEP ステータスを測定します。
現在の PrEP 状態の変化は、1) 現在 PrEP を処方されているか (錠剤または注射)、2) 現在処方を受けているかどうか、3) どの種類/ブランドの PrEP を処方されているかを参加者に尋ねる 3 つの調査項目を使用して測定されます。彼らは現在PrEPを受けています。

フォローアップ時の PrEP 状態の変化 (摂取) は、現在の処方に対する応答範囲のオプションを持つ 3 つの項目を使用して測定され、現在服用している PrEP は、処方された PrEP の種類に対して二値 (はい/いいえ) と公称 (選択肢) です。
参加者が追跡調査時に現在の PrEP 使用を自己報告した場合、研究者は乾燥血液スポット (DBS) サンプリングを使用して PrEP の生物学的存在を確認します。
サンプルは臨床薬理学分析研究所 (CPAL) に輸送され、そこで薬物と代謝物の濃度を分析する FDA 検証済みのアッセイを使用して処理されます。

研究者は、社内で作成したアンケートによる自己申告を使用して、HIV リスク行動の変化を測定します。この機器には、1) 過去 30 日間の注射器の共有のエピソード、および 2) 保護されていない膣または肛門の性交のエピソードを記録するためのいくつかの項目があります。
HIV リスク行動に対する対応オプションは継続的 (エピソードの回数をカウント) であり、使用される HIV 予防技術に対する対応オプションは名目上のものです。例: 「主な性的パートナーとコンドームを使用しました」、「HIV PrEP を受けました」、または「他の人の後に注射器を使用しましたか?」
