水分過剰は死亡リスクと罹患率に悪影響を及ぼし、急性腎不全のリスクを高め、腎代替療法(RRT)の必要性を高め、腎機能の回復を悪化させ、肺損傷(ALI)、感染性合併症を悪化させ、人工肺治療の長期化を引き起こします。換気(APV)、集中治療室(ICU)での滞在期間、および創傷治癒の障害。
重症患者の輸液状態のリアルタイム評価と輸液管理は困難です。 心エコー検査は血行動態の表現型を迅速に特定できますが、連続的な方法ではなく断続的な方法であり、経験と訓練を受けたスタッフが必要です。 半侵襲的方法は、動脈曲線下の面積としての一回拍出量モニタリングと一回拍出量変動 (SVV) の変動に基づいて血管内容積を評価します。 ただし、これらの方法には、間質液、細胞外水 (ECW) の一部、または細胞内液水 (ICW) に関する情報が不足しています。 この問題は、肺血管外水分量 (EVLW) 測定と肺超音波による経肺熱希釈によって部分的に解決されます。 累積バランス (CBF) の計算は、特に不感損失やサードスペース流体損失の流体出力の領域では不正確です。 さらに不正確なのは、末梢浮腫と血流の臨床評価です。 また、体内総水分量 (TBW) のゴールドスタンダードの重水素希釈法は、ICU 環境での日常診療には使用できません。
過剰な水分補給に加えて、重症患者の筋肉組織の急速な喪失は病気の経過に悪影響を及ぼします。 多発性神経筋症は重症患者の最大 40 % に影響を及ぼし、活性化全身性炎症反応 (SIRS) を伴う重度の異化状態にあり、コルチコステロイド療法を受けており、長期人工肺換気ができない患者は危険にさらされています。 除脂肪体重、特に骨格筋量(SMM)をモニタリングすることは依然として困難です。 大腿四頭筋の人体計測や超音波計測は時間がかかり、十分な訓練を受けたスタッフを必要とするため、理想的ではありません。 アルブミンなどの一部の検査パラメータは、炎症 (CRP) や水分補給の影響を受ける可能性があります。 2 つの異なる波長の低強度 X 線を使用するデュアルエネルギー X 線吸収計 (DEXA) は、骨量と軟組織 (除脂肪量、活動量、脂肪) の比較的正確な画像を提供します。 しかし、動けなくなった重症患者に繰り返し X 線検査を行うことは、選択される方法ではありません。
生体電気インピーダンス ベクトル解析 (BIVA) は、特定の組織を流れる変化する電流の流れが水と電解質の含有量に応じて異なるという原理に基づいた、シンプル、迅速、非侵襲的なベッドサイド技術です。 したがって、骨格筋量 (SMM) および体総水分量と細胞外水を含む体細胞量 (BCM) として体組成を測定できます。 また、50 周波数のバイオインピーダンス分光法 (BIS) を使用すると、100 ヘルツ (Hz) より高い周波数の電流のみが細胞膜を通過するため、TBW、ECW、およびそれらの異なる細胞内水を区別することができます。 ただし、この技術では、細胞外コンパートメント内の血管内容積と間質容積を区別することはできません。 多くの研究によると、体組成の生体インピーダンス パラメーターの結果は DEXA に匹敵します。 ただし、BIA は筋肉の表現を過大評価します。 重要なパラメータは位相角 (PA) で、細胞膜を通過する電流の時間遅延、つまり正弦波の電圧波形と電流波形の間の位相シフトを検出します。 PA は BCM を反映し、重要な予後因子として機能します。正常値は 4 ~ 15°です。
臨床検査マーカーのうち、プレセプシン (PSEP) は予後に重要です。 プレセプシン、可溶性分化クラスター 14 (sCD14) は、細菌壁の病原体関連分子パターン (PAMP: リポ多糖、ペプチドグリカン) 部分または細胞へのその他の損傷 - 損傷に応答して、単球およびマクロファージの膜で発現される糖タンパク質です。 -関連分子パターン (DAMP)。 興味深い発見は、多くの研究によって評価された、その予後的役割、つまり生存していない患者の値が高いことです。

ARDSを発症し、ICU入室時に少なくとも7日間の人工肺換気を前提として大学病院の24床のICUに入院した重症成人患者(内科、外傷、外科患者)。
患者の法的責任者からインフォームドコンセントを得た。
説明
包含基準:
- 少なくとも7日間の人工肺換気を前提とした呼吸不全患者(内科、外傷、外科患者)
- 原発性急性呼吸窮迫症候群(ARDS)(肺関与):肺炎、吸入外傷、胸部外傷(肺挫傷)、誤嚥
- 二次性ARDS(肺外):敗血症、ショック状態、急性膵炎、多発性外傷、火傷、非心原性ショック、中毒、TRALI(大量輸血)、溺死
- 慢性閉塞性肺疾患(COPD)の急性増悪患者
除外基準:
- 予後不良の患者
- APACHE II ≥30
- 転移性悪性腫瘍
- 入院前の心肺蘇生(KPCR)後の状態
- 脳浮腫
- 脳外傷
- 頭蓋内圧亢進症
- 肝硬変
- 既存の神経変性疾患
- ペースメーカー、除細動器を使用している患者、妊娠中(生体電気インピーダンスの使用が禁忌の条件)。
