自閉症 - 子供の即興音楽療法の評価 (CHIME)
7~11歳の自閉症児に対する即興音楽療法の有効性に関するランダム化対照試験[Autism-CHIME試験]。
この臨床試験の目的は、7 ~ 11 歳の自閉症児に対する即興音楽療法の個別セッションの有効性を評価することです。
研究者らは、自閉症児に対する通常のケアのみに即興音楽療法を加えた場合の影響を12週間にわたって比較する予定だ。
参加者は、即興音楽療法 (介入) グループまたは通常通りのサポート (対照) グループの 2 つの条件のいずれかにランダムに割り当てられます。
その目的は、次の 7 つの包括的な目標を達成することです。
- 自閉症児の社会的コミュニケーションの改善において、通常のサポートに加えて即興音楽療法の12週間の個別セッションが、通常のサポートのみよりも優れているかどうかを判断する。

- 自閉症児の心理社会的問題の軽減において、通常のサポートに加えて即興音楽療法の12週間の個別セッションが、通常のサポートのみよりも優れているかどうかを検証する。
- 通常のサポートに加えて即興音楽療法の 12 週間の個別セッションが、自閉症の子供の幸福を改善する上で、通常のサポートのみよりも優れているかどうかを検証する。

Autism-CHIME 試験は、自閉症の子供に対する即興音楽療法の個別セッションの厳密なランダム化比較試験 (RCT) として設計されています。 この試験は、ケンブリッジシャー、ピーターバラ、ロンドン、イングランド南部と東部にある普通学校と特別支援学校で実施される。 参加する学校の数は、参加を希望する資格のある児童の数によって異なりますが、1 校あたり最低 5 ~ 10 人の児童が参加します。
登録された参加者は、通常通りのサポートと即興音楽療法セッション(介入群)または通常通りのサポート(対照群)のいずれかに 1 対 1 のブロックでランダム化されます。 ランダム化は、ベースライン評価が完了した後に行われます。 参加者は、割り当てられた BOSCC のバージョン (主な結果測定) に基づいて階層化されます: (1) 最低限の言葉遣い、(2) フレーズの話者、または (3) 流暢な話者のいずれかで、各グループの数が均等になります。そして、対照群と実験群のバランスが取れていること。
データはトライアル中のさまざまな時点で収集されます。
最初のデータ収集ポイント (T1) は、無作為化の前 (参加資格を確立し、ベースライン機能を評価するため) に収集され、主要評価項目 T2 (無作為化後 13 週間、介入終了) で収集され、二次評価項目 T3 は 39 になります。無作為化から数週間後(つまり、音楽療法終了から6か月後)。

- 対象年齢は7歳から11歳。
- ICD-10 基準に従って資格のある専門家によって行われた自閉症の臨床診断。 診断の詳細を記載した臨床報告書のコピー(入手可能な場合)によって確認されるか、子供の両親によって口頭で確認されます。
- 親/保護者は、子供が治験に参加するためにインフォームドコンセントを与える必要があります。
- 保護者は、モニタリングと研究目的で音楽療法セッションと BOSCC 評価をビデオ録画することに同意する必要があります。
- 参加者は、試験期間中、週に 2 回の音楽療法セッションに積極的に参加する必要があります。
- 言葉を話せない子供も含まれる可能性がある
除外基準:
- 治療経過に大きな影響を与える可能性が高いため、前年に定期的に個別音楽療法を受けた。
- 重度の聴覚障害。これにより治療の目的、経過、実施が変更される可能性があります。
- 子どもの心理評価に参加できない保護者。
- 基本的な英語の理解がない介護者。

「通常通りのサポート」に割り当てられた参加者は、一般開業医(GP)、メンタルヘルス、教育/関連医療専門家から日常的なサポートを受けることになります。
通常の支援とは、地域の自閉症専門指導チームによる通常の支援、あるいは自閉症児のコミュニケーションスキルや健康をサポートすることを目的としたその他の追加療法に加えて、各学校の通常の実践として定義されます。
彼らは、研究チームから個別の音楽療法介入や追加のサポートサービスを受けることはありません。
参加する小児が受ける可能性のある併用治療または治療的介入は、無作為化前の評価セッション中に、主要評価項目の後に記録され、介入の種類と量または頻度が特定されます。

実験的:即興音楽療法
即興音楽療法(介入)グループにランダムに割り当てられた参加児童は、12週間にわたって週に2回、30分間の個人音楽療法セッションを受けます。
これらは、学術的に訓練を受け、自閉症の子供たちを扱う臨床経験を持つ、英国の医療専門職評議会 (HCPC) に登録された音楽療法士 (修士レベルまたは同等) によって提供されます。
介入群の各子供は、12週間にわたって24回の音楽療法を受けます。
音楽療法トレーニングマニュアルは、音楽療法士を指導するために使用されます。

セラピストは、子どもの行動や表現に同調しながら、楽器を演奏したり共有したりして子どもと関わったり、歌を歌ったりします。
子どもの興味を引くために、さまざまな即興テクニックが採用されています。
パルス、リズミカル、ダイナミックまたはメロディーのパターン、音色を反映、強化、または補完する機会があり、それによってセラピストと子供の間に同期の瞬間が可能になり、この文脈の中で子供の音楽表現に実用的な意味を与えます。
セラピストはまた、特定の社会的コミュニケーション行動を引き出すために共同で開発された期待やパターンに違反して、子供を穏やかに挑発することもあります。
さらに、子どもには、共同注意、感情の共有、互恵性、歴史の共有、足場、模倣、交代などの社会的コミュニケーションスキルを開発および強化する機会があります。

BOSCC には 3 つの形式があります。1 つは最低限の言語しか話せない人 (MV)、1 つはフレーズスピーチ/若年流暢に話す人 (6 ~ 8 歳までの PS/YF)、そして 1 つは流暢に話す人 (F1/F2教材は 2 セットあり、1 つは子供用、もう 1 つは青少年と大人用です)。
