人工内耳患者と遺伝子治療を受けている DFNB9 患者の間の音声知覚の比較分析:前向き非ランダム化コホート研究
この前向きコホート研究は、人工内耳または遺伝子治療後の患者の多次元的な知覚レベルの傾向と差異を調査するとともに、先天性難聴に対する遺伝子治療の有効性を包括的に評価し、バランスのとれた術後治療を行うための参考を提供することを目的としています。遺伝子治療患者のリハビリテーションプロトコル。

詳細な説明
人工内耳患者は、手術後、静かな環境では音声認識が改善されていますが、騒音下での音楽認識や音声認識はまだ満足のいくものではありません。 遺伝子治療は、自然な聴力を回復する先天性難聴の新規かつ有望な治療法です。 これまで、先天性難聴患者について、遺伝子治療による聴力回復後の、騒音下での音声認識、音楽認識、方向認識などの音を知覚・理解する能力についての研究は行われていない。 さらに、術後の知覚レベルにおける 2 つの治療間の違いは不明のままです。
したがって、研究者らは前向き、非無作為化、単一施設コホート研究を設計した。 介入方法に基づいて、先天性難聴患者は人工内耳グループと遺伝子治療グループの 2 つのグループに分けられます。 本研究では、2 つのグループの本格的な評価が行われます。 このバッテリーには、聴覚言語知覚、認知、心理状態、および聴覚皮質の発達が含まれます。 2つのグループ間の多次元的な違いを調べるために比較分析が行われ、先天性難聴患者に対する遺伝子治療と人工内耳の多様な結果に光が当てられる予定です。

2 つのグループの 1:2 組み合わせによると、遺伝子治療グループの登録者数は 15 名、人工内耳グループの登録者数は 30 名と予想されます。
説明
包含基準:
- 聴力閾値が 65 dB 以上の先天性難聴の患者は、遺伝子治療または人工内耳手術を受けます。
- 年齢は1歳から18歳まで、性別は問いません。
- 母国語は北京語。
- 参加者とその保護者は、治験前にインフォームドコンセントを提供し、自発的に書面による同意書に署名し、指定された時点でのフォローアップを約束しなければなりません。
- 保護者の援助の下で研究者と効果的にコミュニケーションをとることができ、協力して研究者の要求に従う意欲がある。
- 参加者の保護者は、治験について正しく理解し、潜在的な利益について適切な期待を持っている必要があります。
除外基準:
- -前庭蝸牛神経異常、急性/慢性中耳炎、メニエール病、CT/MRIスキャンで明らかになった中耳/内耳の異形成または奇形など、手術結果や研究エンドポイントの解釈を妨げる可能性のある他の耳疾患の存在3ヶ月など
- 他の重度の先天性疾患の存在。
- 重度の全身性疾患の存在、または疾患の急性発症(肺結核、活動性B型またはC型肝炎感染、活動性帯状疱疹感染、膵炎、腎不全など)の存在。
- 免疫力が低い、免疫不全または臓器移植の病歴がある人。
- てんかんや認知症などの神経障害または精神障害の病歴のある人。
- 過去6か月以内の心血管系イベントや脳血管系イベント、予定されている薬剤に対するアレルギーなど、外科医、麻酔科医、または指定職員によって評価された手術または麻酔の禁忌がある患者。
- 研究者が被験者がこの臨床研究に参加するのに不適当であると考えるその他の症状。
