Szczegółowy opis
Po operacji pacjenci z implantem ślimakowym wykazują lepszą percepcję mowy w cichym otoczeniu, ale ich percepcja muzyki i percepcja mowy w hałasie są nadal niezadowalające. Terapia genowa to nowatorska i obiecująca metoda leczenia wrodzonego ubytku słuchu, polegająca na przywróceniu naturalnych wrażeń słuchowych. Do chwili obecnej nie przeprowadzono badań dotyczących zdolności pacjentów z wrodzoną głuchotą do postrzegania i rozumienia dźwięków, takich jak percepcja mowy w hałasie, muzyce i percepcji kierunkowej, po odzyskaniu słuchu dzięki terapii genowej. Ponadto różnica między tymi dwoma sposobami leczenia w zakresie poziomów percepcji po operacji pozostaje nieznana.
Dlatego badacze zaprojektowali prospektywne, nierandomizowane, jednoośrodkowe badanie kohortowe. Ze względu na metodę interwencji pacjentów z wrodzoną głuchotą dzieli się na dwie grupy: grupę implantu ślimakowego i grupę terapii genowej. W niniejszym badaniu przeprowadzona zostanie pełna ocena obu grup. Bateria obejmuje słuchową percepcję mowy, funkcje poznawcze, stan psychiczny i rozwój kory słuchowej. Przeprowadzona zostanie analiza porównawcza w celu zbadania wielowymiarowych różnic między obiema grupami, rzucająca światło na rozbieżne wyniki terapii genowej i implantów ślimakowych u pacjentów z wrodzoną głuchotą.

Kryteria przyjęcia:
- Pacjenci z wrodzonym ubytkiem słuchu i progiem słyszenia ≥65 dB poddawani są terapii genowej lub operacji wszczepienia implantu ślimakowego.
- Wiek od 1 do 18 lat, niezależnie od płci.
- Chiński mandaryński jako język ojczysty.
- Uczestnicy i ich opiekunowie muszą przed rozpoczęciem badania wyrazić świadomą zgodę, dobrowolnie podpisać pisemny formularz zgody i zobowiązać się do dalszych działań w określonych punktach czasowych.
- Potrafi efektywnie komunikować się z badaczami pod opieką opiekuna oraz chce współpracować i spełniać wymagania badaczy.
- Opiekunowie uczestnika powinni właściwie rozumieć przebieg badania i mieć odpowiednie oczekiwania co do potencjalnych korzyści.
Kryteria wyłączenia:
- Obecność innych schorzeń otologicznych, które mogą wpływać na wynik zabiegu chirurgicznego lub interpretację punktów końcowych badania, takich jak nieprawidłowości nerwu przedsionkowo-ślimakowego, ostre/przewlekłe zapalenie ucha środkowego, choroba Meniere’a oraz dysplazja ucha środkowego/wewnętrznego lub wady rozwojowe ujawnione w tomografii komputerowej/MRI w ciągu 3 miesiące itp.
- Obecność innych ciężkich chorób wrodzonych.
- Obecność ciężkich chorób ogólnoustrojowych lub ostry początek chorób, takich jak gruźlica płuc, aktywne zakażenie wirusem zapalenia wątroby typu B lub C, aktywne zakażenie półpaścem, zapalenie trzustki, niewydolność nerek itp.
- Osoby z obniżoną odpornością, niedoborem odporności lub przeszczepem narządu w wywiadzie.
- Osoby, u których w przeszłości występowały zaburzenia neurologiczne lub psychiczne, takie jak epilepsja lub demencja.
- Pacjenci, u których istnieją przeciwwskazania do zabiegu lub znieczulenia ocenione przez chirurga, anestezjologa lub wyznaczony personel, takie jak zdarzenia sercowo-naczyniowe lub mózgowo-naczyniowe w ciągu ostatnich 6 miesięcy, alergia na planowane leki itp.
