化学療法誘発性末梢神経障害(CIPN)に伴う疼痛の軽減における新薬ミロガバリンと従来の治療薬デュロキセチンの有効性を比較するための二群並行並行前向きランダム化対照非盲検試験を実施する。
化学療法誘発性末梢神経障害(CIPN)患者におけるミロガバリン群とデュロキセチン群では、4週間の治療後の痛みの軽減に差が生じます。
参加者は次のことを行います:
- デュロキセチンまたはミロガバリンという薬を 4 週間毎日服用します。
- 2週間に1回のペースで来院していただき、検査・検査を行ってください。

説明
包含基準:
- 19歳以上のがんサバイバー。
- タキサンまたはプラチナ製剤(またはそれらの組み合わせ)による化学療法の開始から 12 週間以内に発生する、両足の末梢の対称的な痛み。
- -NCI有害事象共通用語基準(CTCAE)バージョン5.0で定義される少なくともグレード2の末梢神経障害を経験し、完了後の過去1週間の数値評価スケール(NRS)で平均疼痛強度が4以上の患者化学療法の。
以下の条件が満たされる場合、選択された鎮痛薬 (アセトアミノフェン、非ステロイド性抗炎症薬 [NSAID]) の併用が許可されます。
- 新しい鎮痛剤は導入されません。
- 現在の鎮痛剤は中止されていません。
- 鎮痛薬の毎週 24 時間の総用量は、登録前の 2 週間で 10% を超えて変化しません。
- 末梢神経障害または神経障害性疼痛に対する進行中の治療は、ランダム化の少なくとも 7 日前に中止する必要があります。
除外基準:
- 糖尿病、外傷、アルコール乱用、圧迫、またはその他の原因による末梢神経障害の以前の診断を受けた患者、または以前に中枢神経系障害の診断を受けた患者。 (既存の糖尿病または甲状腺疾患を有し、化学療法前には手や足のしびれやうずきなどの末梢神経障害の症状がなかった患者も含まれる場合があります。)
- 重度のうつ病、双極性障害、自殺念慮などの重度の精神障害のある患者。
- 妊娠中または授乳中の患者。
- 他の神経毒性化学療法剤による以前の治療歴のある患者。
- 腎障害(クレアチニンクリアランス < 30 mL/min)または肝機能障害のある患者。
- 登録後4週間以内に外科手術を予定している患者。

The Brief Pain Inventory-Short Form (BPI-SF) assesses pain severity and pain interference.
Pain severity is assessed using four items and pain interference using seven items.
Each item is rated from 0 to 10.
The pain severity and pain interference scores each range from 0 to 10, with higher scores indicating greater pain severity or greater pain-related interference and therefore a worse outcome.

Chemotherapy-induced peripheral neuropathy (CIPN) severity was assessed using the National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), Version 5.0.
Severity is graded from Grade 1 to Grade 5, with higher grades indicating greater severity; Grade 5 represents death related to the adverse event.

The European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire-Chemotherapy-Induced Peripheral Neuropathy 20-item module (EORTC QLQ-CIPN20) assesses sensory, motor, and autonomic symptoms associated with chemotherapy-induced peripheral neuropathy.
Each item is rated on a 4-point Likert scale ranging from 1 (not at all) to 4 (very much).
Scale scores are linearly transformed to a range of 0 to 100, with higher scores indicating greater neuropathic symptom burden and a worse outcome.

Higher scores on the functional and global health status/quality-of-life scales indicate better functioning or quality of life, whereas higher scores on the symptom scales indicate greater symptom severity and a worse outcome.

Korean Neuropathic Pain Questionnaire (KNPQ)

The Korean Neuropathic Pain Questionnaire (KNPQ) consists of 25 items assessing various characteristics of neuropathic pain.
Twenty-two items are scored on an 11-point numerical rating scale ranging from 0 (no pain) to 10 (the most severe pain imaginable) and are summed to calculate the total score, ranging from 0 to 220.
Higher scores indicate a greater neuropathic pain symptom burden.
