研究者らは、一次検査後に診断を受けていない患者を対象に、遺伝性出血性疾患の診断経路の早期にゲノム検査の導入をテストすることを目的としている。
この臨床試験の目的は、遺伝性出血性疾患の疑いで血液内科に紹介された患者の診断経路の早期にゲノム検査の導入をテストすることです。 回答することを目的とした主な質問は次のとおりです。
- 早期のゲノム検査を追加すると、診断される患者の数は増加しますか?
- 早期にゲノム検査を追加すると、診断までの全体的な時間が短縮されますか?
- 診断経路に早期のゲノム検査を含めることは費用対効果が高いでしょうか?
研究者らは、標準治療を受けている(早期ゲノム検査を受けていない)参加者の対照群と比較する。
参加者は、標準化診断検査と初期ゲノム検査のグループ、または標準化診断検査のみのグループ(研究開始から1年後にゲノム検査が提供される可能性あり)にランダムに割り付けられます。
調査の概要
詳細な説明
現在の標準化された診断検査プロセスでは、重大な出血症状を訴えて紹介された人の最大 50% が、原因不明の出血性疾患 (BDUC) に分類されます。BDUC は、出血スコアが陽性であるものの、現在のすべての診断検査結果が繰り返し正常である患者として定義されます。 。
診断経路の早期にゲノム検査を組み込むことで、診断率が大幅に向上し、診断の遅れが減少し、患者の不安が軽減され、より迅速な症状の認識と標的治療が可能になる可能性があります。

参加者は標準的な診断検査に加えて、早期のゲノム検査を受けることになります。

出血のための遺伝子パネル:この分析では、まれな凝固、血小板、結合組織、および出血障害に関連することが知られている遺伝子のリストを調べます。
現在、パネルには318の遺伝子がありますが、このリストは研究全体で更新される場合があります。
研究の遺伝子には、国際血栓症および血液症協会(ISTH)TIER-1(遺伝子の最初のグループは診断グレード)およびTier-2遺伝子リスト、および公開された研究で同定された追加の遺伝子のものが含まれます。
