詳細な説明
研究チームは、18のモンテフィオーレ診療所で診察中に4〜12歳の420人の子供を対象としたクラスター無作為化比較試験を実施します。 介入群(PRAGMATIC-S)の子供たちは、更新されたガイドラインに基づくケアプロンプトを受け、医療提供者が薬剤投与フォーム(MAF)に記入し、電子署名をしてEHRシステムを通じて直接学校に送付します。 喘息アウトリーチワーカー(AOW)によるケア調整は、家庭と学校での処方された治療の日常的な遵守を支援します。 対照群の子供たちは、強化された通常ケアを受け、これにはガイドラインに基づくケアのEHRプロンプトが含まれますが、追加のPRAGMATIC-Sコンポーネントは含まれません。
参加者は12ヶ月間追跡されます。 結果は、この登録に概説されている通りに評価されます。

対象基準:
- EHRに記録された医師診断による喘息
- 年齢別ガイドラインで定義される持続性またはコントロール不良の喘息(症状が週2日以上、レスキュ―薬使用が週2日以上、夜間症状が月2日以上、または年間2回以上のステロイド投与を要する発作)
- 4~12歳で、NYC公立学校の就学前準備クラスから6年生に在籍
- 保護者が英語またはスペイン語を話せる
- 主たる保護者からの同意および児童(7歳以上の場合)からの承諾
- 電子フォーム入力のための電話及び端末(スマートフォン、iPad、コンピューター)の所持
除外基準:
- 6ヶ月以内に学校/都市を離れる予定の家族
- 重篤な併存疾患(例:先天性心疾患、嚢胞性線維症、慢性肺疾患)
- 里親施設の児童または法的保護者からの同意が得られない状況
- 並行して実施中の喘息介入研究への参加
- ACT質問票の実施を妨げる重度の発達遅延

実験的:プラグマティック-S
プライマリ・ケア・プロバイダー(PCP)は、EHR内のガイドラインベースのプロンプトを使用します。PCPと介護者は、学校で喘息薬を受け取るために、学校に直接送信される電子薬剤投与フォームを電子的に記入します。また、喘息支援ワーカーは、家族、PCP、学校間の連絡役として機能し、ケアの調整とサポートを提供します。

|

|
介護者の生活の質は、検証済みの小児喘息介護者生活の質質問票(PACQLQ; ジュニパー喘息生活の質質問票とも呼ばれる)を用いて評価されます。
この質問票は、子どもの喘息が介護者の生活の質に与える影響を評価する13項目で構成されています。
各項目への回答は1〜7の7段階で評価され、1は「常に」または「非常に非常に心配/不安」、7は「まったくない」または「心配/不安なし」を意味し、全体の可能なスコア範囲は7〜91となります。したがって、高いスコアは喘息関連の生活の質障害が少ないことを示します。

|
喘息コントロールは、小児喘息コントロールテスト(cACT)を用いて二値変数としても測定されます。これは7項目のスケールで、回答コードの合計に基づいてスコアを決定します。 子どもは最初の4つの質問に対して0〜3の4段階尺度で回答し、保護者は最後の3項目に対して0〜5の逆転尺度を用いて回答します。これにより、総合的なスコア範囲は0〜27となり、スコアが高いほど喘息コントロールが良好であることを示します。 喘息コントロールは二値変数として分析され、19点以下のスコアはコントロール不良の喘息を示します。 |

個々の参加者データ (IPD) を共有する予定はありますか?
IPD プランの説明
研究データに加えて、研究プロトコル、データ収集ツール、変数名と形式コードを含むコードブックが提供されます。 量的データセットはSPSS、STATA、および/またはSAS形式で提供されます。 共有されるすべてのデータセットには、関連するメタデータが付属します。 科学的データを操作するには、SAS、STATA、またはSPSSの統計ソフトウェアが必要です。 再現や再利用をサポートするために、共有された科学的データにアクセスしたり操作したりするために、他の専門的なツールは必要ありません。
効率的な使用を促進するために、すべてのデータと資料は以下の標準を使用して構造化および記述されます。縦断的調査の大部分は検証済みの標準化されたツールを使用します。
測定値をスコアリングし、分析のために生データを準備するために使用されるSAS、STATA、またはSPSSコードが提供されます。 医療記録、入院およびEDデータ、電子アドヒアランスモニター、その他すべての測定値のデータは、ベストプラクティスに従って整理および記述されます。

このプロジェクトからの匿名化および/またはグループ化されたデータは、補足資料としてPubMed Central®(PMC)に提出される論文に付随します。PubMed Centralは、原稿提出時(NIH原稿提出システム(NIHMS)を介して)にデータファイル(最大2 GB)を受け入れ、データが公開され、永続的で、引用可能であることを保証します。
データセットは、PubMed Centralに掲載された原稿に関連付けられ、永続的で引用可能なデジタルオブジェクト識別子(DOI)が割り当てられます。
