연구 개요
상세 설명
연구팀은 18개 몬테피오레 진료소에서 내원하는 4-12세 아동 420명을 등록하여 군집 무작위 대조 시험을 수행할 것입니다. 중재 그룹(PRAGMATIC-S)의 아동들은 업데이트된 지침 기반 치료 프롬프트를 받게 되며, 제공자는 약물 투여 양식(MAF)을 작성하고 전자 서명하여 EHR 시스템을 통해 학교로 직접 전송합니다. 천식 아웃리치 워커(AOW)의 치료 조정은 가정과 학교에서 처방된 치료의 일일 순응도를 지원할 것입니다. 대조군의 아동들은 향상된 일반 치료를 받게 되며, 여기에는 지침 기반 치료를 위한 EHR 프롬프트가 포함되지만 추가적인 PRAGMATIC-S 구성 요소는 포함되지 않습니다.
참가자들은 12개월 동안 추적 관찰됩니다. 결과는 이 등록에 명시된 대로 평가될 것입니다.

- 연령별 지침에 따른 지속적이거나 조절되지 않은 천식(증상 주 2일 이상, 긴급 약물 사용 주 2일 이상, 야간 증상 월 2일 이상, 또는 스테로이드 필요 에피소드 연 2회 이상)
- 뉴욕시 공립학교 유치원부터 6학년까지 재학 중인 만 4세에서 12세 아동
- 보호자가 영어 또는 스페인어 구사 가능
- 주 보호자의 동의와 아동의 동의(만 7세 이상인 경우)
- 전자 양식 작성이 가능한 전화 및 기기(스마트폰, 아이패드 또는 컴퓨터) 보유
제외 기준:
- 6개월 이내에 학교/도시를 떠날 계획이 있는 가정
- 중요한 동반 질환(선천성 심장병, 낭포성 섬유증, 만성 폐질환 등)
- 위탁 보호 중이거나 법적 보호자의 동의를 얻을 수 없는 상황의 아동
- 동시 진행 중인 천식 중재 연구에 참여 중인 경우
- ACT 설문지 작성이 불가능한 심각한 발달 지연

실험적: 프라그매틱-S
주치의(PCP)는 EHR 내 지침 기반 프롬프트를 사용하며, 주치의와 보호자는 전자적으로 약물 투약 양식을 작성하여 학교로 직접 전송되어 학교에서 천식 약물을 수령할 수 있도록 하고, 천식 외부 방문 작업자는 가족, 주치의, 학교 간 연락 역할을 수행하며 치료 조정과 지원을 제공합니다.

개입은 EHR 기반 진료 지침 프롬프트, 전자 MAF 제출, 학교 기반 직접 관찰 요법(DOT), 그리고 가족, 학교 및 의료 제공자 간의 진료 조정, 순응도 지원 및 의사소통을 제공하는 천식 관리 지원 전문가(AOWs)를 결합합니다.

보호자의 삶의 질은 검증된 소아 천식 보호자 삶의 질 설문지(PACQLQ; Juniper의 천식 삶의 질 설문지라고도 함)를 사용하여 평가됩니다.
이 설문지는 자녀의 천식이 보호자의 삶의 질에 미치는 영향을 평가하는 13개 항목으로 구성됩니다.
각 항목에 대한 응답은 1-7점 척도로 평가되며, 1은 "항상" 또는 "매우, 매우 걱정됨/염려됨"을 의미하고, 7은 "전혀 없음" 또는 "걱정되지 않음/염려되지 않음"을 의미합니다. 이로 인해 총점 가능 범위는 7-91점이며, 점수가 높을수록 천식 관련 삶의 질 손상이 적음을 나타냅니다.
