PAN-ICIS: 膵切除術後の感染性合併症の早期検出のための集中治療感染スコアを評価する前向き観察研究
この研究は、膵臓手術後の感染を早期に検出するのに役立つ可能性があるICISという新しい血液検査を検討します。
患者は入院中に通常の血液サンプルを採取され、追加の処置は必要ありません。

膵臓切除術は術後の罹病率が高く、特に膵瘻や胆汁漏といった合併症により、しばしば腹腔内感染や敗血症を引き起こします。 従来の炎症マーカー(CRP、WBC、PCT、IL-6)は特異性に欠け、術後のSIRSにより上昇することが多いため、早期診断は依然として困難です。 集中治療感染症スコア(ICIS)は、外科患者においてSIRSと敗血症を区別する際に優れた性能を発揮することが示されています。
PAN-ICIS研究は、膵臓切除術を受ける患者を登録する前向き観察研究です。 ICISは術後に測定され、従来の炎症マーカーと比較されます。 周術期変数、術後合併症、感染性転帰は前向きに収集されます。 SIRSと敗血症の正確な早期鑑別は、膵臓外科における主要なニーズです。 本研究は、術後感染性合併症の早期検出におけるICISの診断精度と臨床的有用性を評価します。 検証されれば、ICISは早期抗菌療法の支援、不必要な抗菌薬曝露の削減、この高リスク集団における術後転帰の改善に寄与する可能性があります。

術後感染性合併症を検出するためのICISの診断精度は、受信者動作特性曲線下面積(AUROC)、感度、特異度、陽性および陰性予測値、および事前に指定されたカットオフ値(例:≥3、≥4、≥5)での尤度比によって測定されます。
