Alvleesklierresecties gaan gepaard met een hoge postoperatieve morbiditeit, vooral door complicaties zoals pancreasfistel en galweg lekkage, die vaak leiden tot intra-abdominale infectie en sepsis. Vroege diagnose blijft moeilijk omdat conventionele ontstekingsmarkers (CRP, WBC, PCT, IL-6) gebrek aan specificiteit hebben en vaak verhoogd zijn door postoperatief SIRS. De Intensive Care Infection Score (ICIS) heeft superieure prestaties getoond in het onderscheiden van SIRS van sepsis bij chirurgische patiënten.
De PAN-ICIS studie is een prospectieve observationele studie die patiënten includeert die alvleesklierresecties ondergaan. ICIS wordt postoperatief gemeten en vergeleken met conventionele ontstekingsmarkers. Perioperatieve variabelen, postoperatieve complicaties en infectieuze uitkomsten worden prospectief verzameld. Accurate vroege differentiatie tussen SIRS en sepsis vertegenwoordigt een grote behoefte in alvleesklierchirurgie. Deze studie zal de diagnostische nauwkeurigheid en klinische bruikbaarheid van ICIS evalueren voor vroege detectie van postoperatieve infectieuze complicaties. Indien gevalideerd, kan ICIS eerdere antimicrobiële therapie ondersteunen, onnodige blootstelling aan antibiotica verminderen en postoperatieve uitkomsten verbeteren in deze hoogrisicopopulatie.

Uitsluitingscriteria:
Patiënten <18 jaar oud. Bekende hematologische maligniteit of hematologische pathologie. Weigering om deel te nemen.

Diagnostische nauwkeurigheid van ICIS voor het detecteren van postoperatieve infectieuze complicaties, gemeten door de oppervlakte onder de receiver operating characteristic curve (AUROC), sensitiviteit, specificiteit, positieve en negatieve voorspellende waarden, en likelihood ratio's bij vooraf gespecificeerde afkapwaarden (bijv. ≥3, ≥4, ≥5).
