Las resecciones pancreáticas conllevan una alta carga de morbilidad postoperatoria, particularmente debido a complicaciones como fístula pancreática y fuga biliar, que frecuentemente conducen a infección intraabdominal y sepsis. El diagnóstico precoz sigue siendo difícil porque los marcadores inflamatorios convencionales (PCR, leucocitos, PCT, IL-6) carecen de especificidad y a menudo se elevan debido al SIRS postoperatorio. El Puntuación de Infección en Cuidados Intensivos (ICIS) ha demostrado un rendimiento superior para distinguir entre SIRS y sepsis en pacientes quirúrgicos.
El estudio PAN-ICIS es un estudio observacional prospectivo que incluye pacientes sometidos a resecciones pancreáticas. El ICIS se medirá postoperatoriamente y se comparará con los marcadores inflamatorios convencionales. Las variables perioperatorias, las complicaciones postoperatorias y los resultados infecciosos se recopilarán prospectivamente. La diferenciación precoz precisa entre SIRS y sepsis representa una necesidad importante en la cirugía pancreática. Este estudio evaluará la precisión diagnóstica y la utilidad clínica del ICIS para la detección temprana de complicaciones infecciosas postoperatorias. Si se valida, el ICIS podría respaldar una terapia antimicrobiana más temprana, reducir la exposición innecesaria a antibióticos y mejorar los resultados postoperatorios en esta población de alto riesgo.
