この前向き連続観察研究は、TDN Clínica(スペイン、ナバラ州)の通常外来診療で実施され、2025年3月から9月の間に同一の標準化されたデジタルワークフロー(REHABI)の下で管理された非外傷性肩痛/機能障害を有する成人に対して、2つの理学療法提供形式間でアウトカムに差があるかどうかを評価することを目的とした。 形式への割り当ては、通常診療上の制約(例:勤務スケジュール、交通手段、距離)に基づく実現可能性(非無作為)によるものであった。 両形式とも、名目上の頻度は週3回で実施された:ハイブリッド形式(1回の指導付き対面セッション+処方された運動へのスマートフォンアクセスでサポートされる2回の非指導在宅セッション)と完全対面形式(週3回の指導付き対面セッション)。
REHABIは、構造化された評価-処方-再評価プロセスを統合している。 患者は、ベースライン(初回受診時)および臨床的退院まで3~5週間ごとにスケジュールされたフォローアップ評価時に、標準化された24項目機能バッテリーを完了した。 このバッテリーには、筋力(デジタルダイナモメトリー)、可動域(手動ゴニオメトリー)、柔軟性、臨床テストの測定が含まれる。 テスト実施中の疼痛強度は、24テスト中16テストにおいて0-10の視覚的アナログ尺度(VAS/EVA)を用いて記録された;複合VASも退院判断を支援するために使用された。 テストスコアリングにより、総臨床欠損スコアが算出される。 臨床的退院は、ワークフロー内で明示的な基準(欠損スコア閾値、対称的筋力、機能的可動域、テスト中の低疼痛)を用いて定義された。
研究目的:(1) 通常診療において、ハイブリッド形式と完全対面リハビリテーションで管理された患者間で、臨床的退院までの時間(ベースラインから退院までの日数)に差があるかどうかを評価すること。 (2) 記録された疼痛(VAS/EVAおよび複合VAS)および総臨床欠損スコアのベースラインから退院までの変化が、形式間で異なるかどうかを評価すること。

- 関節炎/変形性関節症
- 骨肉腫または骨髄炎
- 最近の肩の手術

This outcome was analyzed among rehabilitation episodes with a documented discharge event within the predefined study window.

The total clinical deficit score was calculated from the standardized 24-test shoulder assessment and ranged from 0 to 36 points, with higher scores indicating greater clinical impairment.
Change was calculated as baseline score minus discharge score; therefore, positive values indicate a reduction in clinical deficit.
The analysis included rehabilitation episodes with paired baseline and documented discharge assessments.

Pain during test performance was recorded using a 0-10 visual analogue scale (VAS), where 0 indicates no pain and 10 indicates the greatest pain intensity.

Lower values indicate less pain.

In the hybrid group, prescribed sessions included one supervised in-clinic session and two unsupervised home-based sessions per week.
In the in-clinic group, prescribed sessions included three supervised in-clinic sessions per week.
Higher percentages indicate greater adherence.
