米国では毎年、200万~300万人の重症成人患者が侵襲的機械的人工呼吸を受けています。 機械的人工呼吸を受けている重症成人患者の院内死亡率は依然として約25~35%です。 侵襲的機械的人工呼吸を受けている重症成人患者の死亡率を低下させるケアのアプローチが緊急に必要とされています。
機械的人工呼吸では、動脈血酸素飽和度(パルスオキシメトリー(SpO2)または血液ガス分析(SaO2)で評価)または動脈血酸素分圧(例:PaO2)を維持するために、吸入酸素濃度(FiO2)を調整することが普遍的です。 より高いSpO2目標値(96~100%)を使用すると、低酸素血症に対する安全マージンが得られますが、過剰なFiO2、高酸素血症、組織高酸素への曝露が増加し、酸化ストレスや炎症を引き起こす可能性があります。 より低いSpO2目標値(88~92%)を使用すると、これらのリスクを最小限に抑えられますが、低酸素血症や低酸素誘導性臓器障害への曝露が増加する可能性があります。 歴史的に、より高いSpO2目標値とより低いSpO2目標値が患者の転帰に及ぼす影響は不明でした。
内科集中治療室(ICU)で機械的人工呼吸を受けている2,541人の重症成人患者を対象に、より高いSpO2目標値とより低いSpO2目標値を比較した私たちの最近の無作為化試験では、より高いSpO2目標値の使用は、より低いSpO2目標値の使用と比較して、短期転帰(例:28日死亡率)または長期転帰(例:12か月時の認知機能)に全体的な差をもたらさないことがわかりました(Semler, NEJM, 2022; Mart, AJRCCM, 2024)。 結果は、異なる環境で実施された他の複数の大規模無作為化試験でも同様でした。
無作為化試験は伝統的に、全体集団に対する治療の平均効果を転帰について報告します。 しかし、治療が転帰に及ぼす効果は、患者の特性が異なれば異なる可能性があります。 このような治療効果の大きさや方向の非無作為的な変動は、治療効果の不均一性と呼ばれます。 特定の患者にとってどの治療が最良の転帰をもたらすかを理解するために、臨床医と患者は、平均治療効果の報告を超えて、患者の独自の特性に基づいて個々の患者の転帰に対する治療効果を報告する無作為化試験を必要としています。これは個別化治療効果と呼ばれます。
最近、私たちは、より高いSpO2目標値とより低いSpO2目標値を比較した試験のデータセットを使用して、各患者のベースライン特性を同時に考慮し、個々の患者の28日死亡率に対する、より高いSpO2目標値とより低いSpO2目標値の使用の効果を予測する統計モデルを開発しました(Buell, JAMA, 2024)。この統計モデルは、侵襲的機械的人工呼吸開始時に利用可能な24の患者特性を使用して、その患者にとって最良の転帰をもたらすSpO2目標値を予測します。 モデルへの入力は、24のベースライン特性それぞれに対する各患者の値です。 モデルの出力は、患者のすべてのベースライン特性の値に条件付けられた、その患者の28日院内死亡率に対する、より高いまたはより低いSpO2目標値を使用する予測絶対効果です。 この統計モデルの精度を検証するために、私たちはそれを、地理的および時間的に異なる、より高いSpO2目標値とより低いSpO2目標値を比較した第2の無作為化試験(Mackle, NEJM, 2019)のデータセットに適用しました。 その結果、どちらの試験でも平均治療効果に有意差はなかったにもかかわらず、死亡率に対するより高いSpO2目標値とより低いSpO2目標値の使用の効果は、個々の患者によって大きく異なり、多くの患者がより低いまたはより高いSpO2目標値のいずれかから利益を得ているように見えることがわかりました。
この統計モデルが臨床ケアで広く適用される前に、モデルからの情報を使用して酸素療法を指導することが、通常ケアと比較して患者の転帰を改善するかどうかを決定するための無作為化試験が必要です。 EXPRESS試験は、バンダービルト大学のICUで機械的人工呼吸を受けている成人を対象に、個別化SpO2目標群(臨床医が各患者にとって最良の転帰をもたらすと予測されるSpO2目標値に関する情報を受け取る)と通常ケア群(臨床医が各患者にとって最良の転帰をもたらすと予測されるSpO2目標値に関する情報を受け取らない)を比較する無作為化試験となります。

パーソナライズされたSpO2目標群の患者に対しては、電子健康記録内の統計モデルが各患者のベースライン特性を使用し、個々の患者にとって最良の結果をもたらすと予測されるSpO2目標を計算します。高いSpO2目標が有益と予測される患者に対しては98%(範囲:96-100%)、低いSpO2目標が有益と予測される患者に対しては90%(範囲:88-92%)となります。
患者にとって最良の結果をもたらすと予測されるパーソナライズされたSpO2目標は、医師、看護師、呼吸療法士によって、日常的な臨床ケアの一環として提供されます。

The nonrespiratory Sequential Organ Failure Assessment (SOFA) score is composed of scores from five organ systems (excluding the respiratory system), graded from 0 to 4 according to the degree of dysfunction or failure.
Scores range from 0 (no evidence of nonrespiratory organ dysfunction or failure) to 20 (evidence of severe nonrespiratory organ dysfunction or failure).
