이 연구의 목적은 화학방사선 요법 전과 도중에 운동 처방 준수를 촉진하는 원격 감독 운동 프로그램(REM)의 가능성과 잠재적 효과를 평가하는 파일럿 무작위 통제 시험(RCT)을 수행하는 것입니다.

이것은 예비 연구이며 치료 효과에 대한 확실한 증거를 제공하기 위해 고안된 것이 아니라 실행 가능성을 확립하고 더 큰 시험에 대비하여 개입 및 연구 절차를 최적화하기 위해 고안되었습니다.
원격 감독 운동 프로그램(REM)과 감독되지 않는 운동 프로그램(UNSUP)을 비교하는 시범 RCT를 실시할 것입니다.
데이터는 등록 시(T0; 화학방사선 요법 시작 최소 2주 전), 화학방사선 요법 직전(T1), 화학방사선 요법 직후(T2) 및 화학방사선 요법 후 1개월(T3)에 수집됩니다.

- 18세 이상이고 IIa기에서 IIIb기 폐암 진단을 받았습니다.
- 방사선과 병행하여 매주 카보플라틴 및 파클리탁셀을 사용한 화학방사선 요법을 통한 최종 치료는 2주 이상 후에 시작할 계획입니다.
- 피트니스 장치 앱을 설치하고 WiFi 또는 셀룰러 서비스에 액세스할 수 있는 Apple 또는 Android 장치가 있어야 합니다.
- 영어를 구사하고 자발적인 서면 동의를 제공할 수 있습니다.
- Zubrod/ECOG 수행 상태 0-1로 표시되는 화학방사선 요법을 견딜 수 있음; 연구 등록 전 14일 이내에 획득한 CBC/감별, 다음과 같이 정의된 적절한 골수 기능: 절대 호중구 수(ANC) ≥ 1,500 세포/mm3; 혈소판 ≥ 100,000 세포/mm3; 헤모글로빈 ≥ 8.0g/dl; 등록 전 14일 이내에 적절한 신장 기능(크레아티닌 청소율로 정의됨)은 최소 35ml/min이어야 합니다. 기관의 경우 총 빌리루빈 ≤ 1.5 x 정상 상한(ULN)으로 정의되고 기관의 경우 ALT, AST 및 알칼리 포스파타제 ≤ 2.5 x ULN으로 정의되는 등록 전 14일 이내에 적절한 간 기능; 이전 흉부 방사선 요법 없음;
제외 기준:
- 기대 수명이 12개월 미만이거나 호스피스 서비스를 받고 있습니다.
- 통제되지 않는 심각한 정신 질환, 약물 남용 또는 능동적 자살 관념과 같은 필요를 충족시키기 위해 상당한 연구 수정이 필요한 정신과 진단;
- 미국 스포츠 의학 대학(American College of Sports Medicine)은 >120bpm의 휴식 심박수, >180/100mmHg의 혈압 또는 불안정 협심증 40 또는 운동을 예방하는 근골격계 문제를 포함하는 운동에 대한 금기 사항을 보여줍니다.
- 100미터를 걸을 수 없습니다. 현재 치료 표준에 따르면 해당 참가자는 평가 및 치료를 위해 물리 치료를 받고 감독되지 않은 운동은 안전하지 않기 때문에 연구에서 제외됩니다.
- 화학방사선 요법 시작까지 2주 미만;
- 의사 재량;
- 걸을 수 없거나 6분 걷기 테스트를 완료할 수 없습니다. 현재 치료 표준에 따르면 해당 참가자는 평가 및 치료를 위해 물리 치료를 받게 되며 감독되지 않은 운동은 안전하지 않기 때문에 연구에서 제외됩니다.

신체 활동 수용 설문지(PAAQ)61는 운동의 맥락에서 신체적, 심리적 불편함을 수용하는 개인의 능력을 측정하는 데 사용되는 10개 항목 척도입니다.
PAAQ의 모든 항목은 점수가 매겨진 7점 리커트 척도(1 = 매우 동의하지 않음 ~ 7 = 매우 동의함)로 평가됩니다.
심리적 유연성은 어떤 식으로든 경험을 수정, 억제 또는 종료하려는 시도 없이 현재 경험(즉, 자신의 생각과 감정)에 개방적인 자세입니다.
