트레드밀 기반 로보틱스와 전통적인 보행 훈련에 의한 뇌졸중 후 보행 재활의 효과 (TREAD_STROKE)
뇌졸중 생존자에서 러닝머신 기반 로봇(외골격 또는 엔드 이펙터)을 사용한 보행 재활 대 기존 물리 치료의 효과: 다기관 제어 비무작위 시험
본 다기관 비무작위 대조시험은 보행 후 보행 회복을 위한 기존 재활 치료와 비교하여 외골격 또는 엔드 이펙터 시스템을 이용한 로봇 치료의 효과(10미터 보행 테스트에서 보행 속도 증가 - 10MWT)를 조사하는 것을 목표로 합니다. 다양한 뇌졸중 후 장애 프레임워크에서 엔드 이펙터 및 외골격 시스템의 가능한 다른 효능을 비교합니다.
아급성 또는 만성 단계 모두에서 재활 센터에 입원한 적격 피험자는 모두 기록됩니다. 실험 그룹은 지상 보행 훈련을 제공하지 않는 러닝머신 기반 로봇 시스템(Lokomat Pro - Hocoma AG, Volketswil, Switzerland; G-EO System - Reha technologies, Italy)에서 일련의 로봇 보행 훈련을 따를 것입니다. 반면 통제 그룹은 보행 능력 회복을 촉진하는 모든 운동으로 구성된 전통적인 보행 훈련을 따를 것입니다(개입 세부 사항 참조).

상세 설명
이 다기관 비무작위 대조 시험은 다음을 목표로 합니다.
- 뇌졸중 생존자의 보행 회복에 대한 기존 재활 치료와 비교하여 트레드밀 기반 로봇 치료(엔드 이펙터 및 외골격 모두)의 효능을 조사합니다.
- 장애가 다른 뇌졸중 대상자에 대한 엔드 이펙터 또는 외골격 시스템의 다양한 효과를 확인합니다.
재활 치료를 위해 연구 참가자 센터에 입원한 모든 적격 피험자(포함 및 제외 기준 참조)는 재활 프로그램의 모든 단계(아급성 또는 만성 단계)에서 기록됩니다.
환자의 모집은 12개월 동안 각 윤리 위원회의 승인에 따라 진행됩니다.
실험군과 대조군 모두 일주일에 3~5회, 60분씩 총 20회의 치료를 받게 됩니다. (총 400분의 치료) - Hocoma AG(스위스 Volketswil, G-EO System - Rehatechnologies, 이탈리아)는 트레드밀 기반이며 지상 보행 훈련을 제공하지 않습니다. 그들은 속도, 보폭뿐만 아니라 프로그래밍 가능한 부하 서스펜션의 가능성이 특징입니다. 관련 데이터와 함께 환자의 활동은 항상 컴퓨터 제어 시스템에 표시되고 저장됩니다.

- 로봇 운동을 올바르게 수행하기 위한 간단한 지침을 이해하고 실행할 수 있는 가능성;
- 만성 환자의 경우: 기능적 보행 범주(FAC) > 1.
제외 기준:
- 양측 장애;
- 급성 사건 전 WHS(Walking Handicap Scale) <5;
- 로봇 훈련의 이해를 손상시키는 인지 또는 행동 결함;
- 이전 3개월 동안 및/또는 연구 동안(추적 포함) 보툴리눔 독소를 사용한 신경용해 치료;
- 연구 중 다른 기술(로봇, FES, TDCS ...) 사용;

러닝머신 기반 로봇 보행 훈련
러닝머신 기반 로봇 보행 훈련(TRGT) 기간은 전체적으로 최소 400피트의 운동을 위해 주당 3-5일, 20개의 세션을 지속합니다.
로봇 훈련을 위해 준수해야 할 매개변수는 모든 환자에 대해 다음과 같습니다: 0.9km/h 시작 속도 최대 2.5km/h; 처음에는 체중의 40-45%를 초과하지 않는 체중 지원 및 사례에 따라 점진적인 점진적 감소; Lokomat의 경우: 치료 시작 시 필요한 최대 지원 및 치료 중 점진적 감소.
TRGT는 항상 전통적인 보행 재활과 연관될 것이며 일반적으로 아급성기 환자를 위한 3시간의 재활 치료, 만성기 환자를 위한 60' 치료를 포함하는 개별 재활 프로젝트의 일부가 될 것입니다.

환자는 자신이 선호하는 최대 안전 속도로 걸을 것을 요청받게 됩니다.
환자는 출발선 1m 앞에 위치하여 10m를 걷고 약 1m 후에 종점을 통과하도록 지시받습니다.
코스 전후 거리는 가감속의 영향을 최소화하기 위한 것이다.
시간은 스톱워치를 사용하여 측정되며 100분의 1초(예: 2.15초)까지 기록됩니다.
테스트는 3번 기록되며 중간에 적절한 휴식을 취합니다.

6MWT는 필요에 따라 보조 장치를 사용하여 6분 동안 평평하고 단단한 표면에서 피험자가 걷는 거리를 측정합니다.
이 테스트는 기능적 운동 능력에 대한 신뢰할 수 있고 유효한 평가이며 유산소 능력 및 지구력의 준최대 테스트로 사용됩니다.
아급성 뇌졸중 환자의 감지 가능한 최소 거리 변화는 60.98미터입니다.
6MWT는 환자의 자가 보행 검사로 기능적 능력 수준을 평가합니다.
환자는 검사 중에 멈추고 휴식을 취할 수 있습니다.
그러나 타이머는 멈추지 않습니다.
환자가 테스트를 완료할 수 없으면 그 순간 시간이 중지됩니다.
누락된 시간과 중지 이유가 기록됩니다.
이 테스트는 심박수와 산소 포화도를 모니터링하기 위해 산소 포화도 측정기를 착용한 상태에서 실시되며 호흡곤란을 평가하기 위해 Borg 척도와 통합됩니다.
