상세 설명
1차 편측 슬관절 전치환술 환자에서 조사자는 임상 골다공증 환자에서 수술 전 3개월부터 시작하여 총 15개월 동안 지속되는 일일 아발로파라타이드 요법의 효과를 조사할 것입니다.
이것은 슬관절 전치환술(TKA) 후 12개월 동안 치료받지 않은 골다공증 환자에서 이전에 발표된 값뿐만 아니라 치료를 받지 않은 골감소증 환자와 비교될 것입니다.

포함 기준
- 55세 이상의 폐경 후 여성 및 남성으로 위스콘신 대학교 종합 관절 프로그램에서 기본 TKA를 받을 예정입니다.
- 골다공증, 즉, BMD T-점수(여성 참고 자료 사용) 요추, 대퇴 경부 또는 전체 고관절에서 ≤ -2.5 또는 척추 골절 평가에서 척추 골절이 확인된 ≤ -1.1 또는 지난 5년 동안의 저외상 비척추 골절 병력 년 또는 골감소증, 요추, 대퇴 경부 또는 전체 고관절에서 BMD T-점수(여성 참조 데이터 사용) -1.1 ~ -2.4 및 이전의 저외상 골절 없음.
- 혈청 칼슘(알부민 보정), 혈청 크레아티닌 및 부갑상선(PTH) 값은 모두 정상 범위 내에 있고 25(OH)D > 10ng/mL입니다.
- 프로토콜에 지정된 용량으로 매일 칼슘 및/또는 비타민 D3를 보충하고자 합니다.
- 서면 동의서를 제공할 수 있습니다.
제외 기준
- 하드웨어 또는 기타 인공물로 인해 평가할 수 없는 원위 대퇴골 BMD.
- 골다공증 이외의 뼈 장애(예: 파제트병)의 병력.
- 방사성 요오드 이외의 골격을 포함하는 이전의 외부 빔 또는 임플란트 방사선 요법의 병력.
- 만성 또는 재발성 신장, 간, 폐, 알레르기, 심혈관, 위장관, 내분비, 중추 신경계, 혈액 또는 대사 질환, 또는 연구 책임자의 의견에 따라 연구 데이터를 손상시킬 수 있는 면역학적, 정서적 및/또는 정신 장애의 병력 타당성.
- 지난 1년 이내에 쿠싱병, 성장 호르몬 결핍 또는 과잉, 갑상선 기능 항진증, 부갑상선 기능 저하증 또는 기능 항진증 또는 흡수 장애 증후군의 병력.
- 현저하게 손상된 신장 기능의 병력(혈청 크레아티닌 >2.0 mg/dL. 혈청 크레아티닌이 > 1.5 및 ≤ 2.0 mg/dL인 경우 계산된 크레아티닌 청소율(Cockcroft-Gault)은 ≥ 37 mL/min이어야 합니다.
- 지난 5년 이내에 신결석증 또는 요로결석증의 병력.
- 이전 5년 동안의 암 병력(기저 세포 또는 편평 피부암은 허용됨).
- 언제든지 골육종의 병력.
- B형 간염, C형 간염, HIV-1 또는 HIV-2 양성인 것으로 알려진 환자.

- 언제든지 정맥 비스포스포네이트 또는 지난 3년 이내에 경구 비스포스포네이트를 사용한 이전 치료. 단기간 경구용 비스포스포네이트 요법(3개월 이하)을 받은 환자는 치료가 등록 전 6개월 이상 발생한 경우 등록할 수 있습니다.
- 지난 5년 동안 불소 또는 스트론튬으로 치료했거나 언제든지 뼈에 작용하는 연구용 제제로 치료를 받은 적이 있는 자.
- 지난 6개월 동안 칼시토닌 또는 지난 18개월 동안 데노수맙으로 치료.
- 이전 6개월 이내에 비타민 D 대사에 영향을 미치는 항경련제(페노바르비탈, 페니토인, 카르바마제핀 또는 프리미돈) 또는 만성 헤파린 치료.
- 아나볼릭 스테로이드 또는 칼시뉴린 억제제(시클로스포린, 타크로리무스)로 치료
- 이전 12개월 동안 경구, 비강 또는 흡입 글루코코르티코이드를 사용한 매일 치료.
- 12개월 이내에 시험용 약물에 노출된 경우.

Knee Injury & Osteoarthritis Outcome Score (KOOS) JR

Patient reported knee function score.
The KOOS, JR was developed from the original long version of the Knee injury and Osteoarthritis Outcome Score (KOOS) survey using Rasch analysis.
The KOOS, JR contains 7 items from the original KOOS survey.
Items are coded from 0 to 4, none to extreme respectively.
KOOS, JR is scored by summing the raw response (range 0-28) and then converting it to an interval score (0-100).
The interval score ranges from 0 to 100 where 0 represents total knee disability and 100 represents perfect knee health.

Precision Error on Knee Bone Density Measurement

|
Precision assessment in the field of bone densitometry is the process whereby the ability of the instrument and the technologist to reproduce similar results, given no real biologic change, is tested.
To achieve statistical power, the investigators will take duplicate knee bone density measurements on 30 participants at either their 6 month or 15 month visit.
The standard deviation for each participant is calculated, then the root mean square standard deviation for the group is calculated.
The mathematical result of precision assessment is displayed as percent coefficient of variation (CV).
