Kryteria przyjęcia
- Kobiety i mężczyźni po menopauzie w wieku ≥ 55 lat, którzy mają przejść pierwotną TKA w Total Joint Programme University of Wisconsin.
- Osteoporoza, tj. BMD T-score (na podstawie danych referencyjnych dla kobiet) ≤ -2,5 w odcinku lędźwiowym kręgosłupa, szyjce kości udowej LUB w całym biodrze lub ≤ -1,1 z oceną złamania kręgów potwierdzoną złamaniem kręgów lub złamaniem kręgów bez urazów w wywiadzie w ciągu ostatnich 5 lat LUB osteopenia, BMD T-score (na podstawie danych referencyjnych kobiet) -1,1 do -2,4 w odcinku lędźwiowym kręgosłupa, szyjce kości udowej lub całym biodrze i brak wcześniejszych złamań o niskim urazie.
- Stężenie wapnia w surowicy (z uwzględnieniem albumin), kreatyniny w surowicy i wartości przytarczyc (PTH) mieszczą się w prawidłowym zakresie i 25(OH)D > 10 ng/ml.
- Chęć codziennego uzupełniania wapnia i/lub witaminy D3 w dawkach określonych w protokole.
- Zdolność do wyrażenia pisemnej świadomej zgody.
Kryteria wyłączenia
- Nieoceniona BMD dystalnej części kości udowej z powodu sprzętu lub innych artefaktów.
- Historia chorób kości (np. choroba Pageta) innych niż osteoporoza.
- Historia wcześniejszej radioterapii wiązką zewnętrzną lub implantem, obejmującej szkielet inny niż jod promieniotwórczy.
- Historia przewlekłych lub nawracających chorób nerek, wątroby, płuc, alergii, układu sercowo-naczyniowego, żołądkowo-jelitowego, endokrynologicznego, ośrodkowego układu nerwowego, hematologicznych lub metabolicznych lub zaburzeń immunologicznych, emocjonalnych i/lub psychiatrycznych, które w opinii głównego badacza mogłyby naruszyć dane z badania ważność.
- Historia choroby Cushinga, niedoboru lub nadmiaru hormonu wzrostu, nadczynności tarczycy, niedoczynności lub nadczynności przytarczyc lub zespołów złego wchłaniania w ciągu ostatniego roku.
- Znaczne upośledzenie czynności nerek w wywiadzie (stężenie kreatyniny w surowicy >2,0 mg/dl. Jeśli stężenie kreatyniny w surowicy wynosi > 1,5 i ≤ 2,0 mg/dl, obliczony klirens kreatyniny (Cockcroft-Gault) musi wynosić ≥ 37 ml/min.
- Historia kamicy nerkowej lub kamicy moczowej w ciągu ostatnich pięciu lat.
- Historia choroby nowotworowej w ciągu ostatnich 5 lat (dopuszczalny jest rak podstawnokomórkowy lub płaskonabłonkowy skóry).
- Historia kostniakomięsaka w dowolnym momencie.
- Pacjenci, u których stwierdzono wirusowe zapalenie wątroby typu B, wirusowe zapalenie wątroby typu C, HIV-1 lub HIV-2.
- Znana nadwrażliwość na którykolwiek z badanych materiałów lub pokrewne związki.
- Wcześniejsze leczenie lekami będącymi pochodnymi PTH lub PTHrP (ABL, teryparatyd lub PTH (1-84)).
- Wcześniejsze leczenie bisfosfonianami dożylnymi w dowolnym czasie lub bisfosfonianami doustnymi w ciągu ostatnich trzech lat. Pacjenci, którzy otrzymali krótki cykl doustnej terapii bisfosfonianami (3 miesiące lub krócej), mogą zostać włączeni, o ile leczenie miało miejsce 6 lub więcej miesięcy przed włączeniem.
- Leczenie fluorem lub strontem w ciągu ostatnich pięciu lat lub wcześniejsze leczenie badanymi czynnikami działającymi na kości w dowolnym momencie.
- Leczenie kalcytoniną w ciągu ostatnich 6 miesięcy lub denosumabem w ciągu ostatnich 18 miesięcy.
- Leczenie lekami przeciwdrgawkowymi wpływającymi na metabolizm witaminy D (fenobarbital, fenytoina, karbamazepina lub prymidon) lub przewlekła heparyna w ciągu ostatnich 6 miesięcy.
- Leczenie sterydami anabolicznymi lub inhibitorami kalcyneuryny (cyklosporyna, takrolimus)
- Codzienne leczenie glikokortykosteroidami doustnymi, donosowymi lub wziewnymi w ciągu ostatnich 12 miesięcy.
- Ekspozycja na jakikolwiek badany lek w ciągu 12 miesięcy.
- Spożywanie > 2 drinków alkoholowych dziennie lub zażywanie narkotyków w ciągu 12 miesięcy od badania przesiewowego.
- Nie nadaje się do udziału w badaniu z innych powodów według uznania badacza.

Knee Injury & Osteoarthritis Outcome Score (KOOS) JR

Patient reported knee function score.
The KOOS, JR was developed from the original long version of the Knee injury and Osteoarthritis Outcome Score (KOOS) survey using Rasch analysis.
The KOOS, JR contains 7 items from the original KOOS survey.
Items are coded from 0 to 4, none to extreme respectively.
KOOS, JR is scored by summing the raw response (range 0-28) and then converting it to an interval score (0-100).
The interval score ranges from 0 to 100 where 0 represents total knee disability and 100 represents perfect knee health.

Precision Error on Knee Bone Density Measurement

|
Precision assessment in the field of bone densitometry is the process whereby the ability of the instrument and the technologist to reproduce similar results, given no real biologic change, is tested.
To achieve statistical power, the investigators will take duplicate knee bone density measurements on 30 participants at either their 6 month or 15 month visit.
The standard deviation for each participant is calculated, then the root mean square standard deviation for the group is calculated.
The mathematical result of precision assessment is displayed as percent coefficient of variation (CV).
