Kuvaus
Sisällyttämiskriteerit
- Postmenopausaaliset naiset ja miehet ovat iältään ≥ 55-vuotiaita ja heille on määrä suorittaa ensisijainen TKA Wisconsinin yliopiston yhteisohjelmassa.
- Osteoporoosi, eli luun mineraalitiheyden T-pisteet (naisen vertailutietoja käyttäen) ≤ -2,5 lannerangassa, reisiluun kaulassa TAI koko lonkassa tai ≤ -1,1 nikamamurtumaarvioinnissa vahvistivat nikamamurtuman tai aiemman vähävamman aiheuttaneen nonvertebral-murtman 5 vuotta TAI osteopenia, luun mineraalitiheyden T-pisteet (naisten vertailutietoja käyttäen) -1,1 - -2,4 lannerangassa, reisiluun kaulassa tai koko lonkassa, eikä aikaisempaa lievää traumaa aiheuttavaa murtumaa.
- Seerumin kalsium (albumiinikorjattu), seerumin kreatiniini- ja lisäkilpirauhasarvot (PTH) ovat kaikki normaalialueella ja 25(OH)D > 10 ng/ml.
- Valmis täydentämään päivittäistä kalsiumia ja/tai D3-vitamiinia protokollan määrittämillä annoksilla.
- Pystyy antamaan kirjallinen tietoinen suostumus.
Poissulkemiskriteerit
- Arvioimaton distaalinen reisiluun BMD laitteiston tai muiden artefaktien vuoksi.
- Aiemmin muita luusairauksia (esim. Pagetin tauti) kuin osteoporoosia.
- Aiempi ulkoinen säde- tai implanttisäteilyhoito, johon on osallistunut muu kuin radiojodi.
- Krooniset tai toistuvat munuaisten, maksan, keuhkojen, allergisten, sydän- ja verisuonisairauksien, maha-suolikanavan, endokriinisten, keskushermoston, hematologisten tai aineenvaihduntasairauksien historia tai immunologiset, tunneperäiset ja/tai psykiatriset häiriöt, jotka päätutkijan mielestä vaarantaisivat tutkimustuloksia pätevyyttä.
- Cushingin tauti, kasvuhormonin puutos tai liikakasvu, kilpirauhasen liikatoiminta, hypo- tai hyperparatyreoosi tai imeytymishäiriöt viimeisen vuoden aikana.
- Aiempi merkittävästi heikentynyt munuaisten toiminta (seerumin kreatiniini > 2,0 mg/dl. Jos seerumin kreatiniini on > 1,5 ja ≤ 2,0 mg/dl, lasketun kreatiniinipuhdistuman (Cockcroft-Gault) on oltava ≥ 37 ml/min.
- Aiempi munuaiskivitauti tai virtsakivitauti viimeisen viiden vuoden aikana.
- Aiempien 5 vuoden aikana esiintynyt syöpä (tyvisolu- tai levyepiteelisyöpä on sallittu).
- Osteosarkooman historia milloin tahansa.
- Potilaat, joiden tiedetään saaneen hepatiitti B-, C-hepatiitti-, HIV-1- tai HIV-2-positiivisen.
- Tunnettu yliherkkyys jollekin testimateriaalille tai läheisille yhdisteille.
- Aiempi hoito PTH- tai PTHrP-peräisillä lääkkeillä (ABL, teriparatidi tai PTH (1-84)).
- Aiempi hoito suonensisäisillä bisfosfonaateilla milloin tahansa tai oraalisilla bisfosfonaateilla viimeisen kolmen vuoden aikana. Potilaat, jotka ovat saaneet lyhyttä oraalista bisfosfonaattihoitoa (3 kuukautta tai vähemmän), voidaan ottaa mukaan niin kauan kuin hoito on suoritettu vähintään 6 kuukautta ennen osallistumista.
- Hoito fluorilla tai strontiumilla viimeisen viiden vuoden aikana tai aiempi hoito luuhun vaikuttavilla tutkimusaineilla milloin tahansa.
- Kalsitoniinihoito viimeisen 6 kuukauden aikana tai denosumabilla viimeisen 18 kuukauden aikana.
- Hoito D-vitamiinin aineenvaihduntaan vaikuttavilla kouristuslääkkeillä (fenobarbitaali, fenytoiini, karbamatsepiini tai primidoni) tai krooninen hepariini viimeisten 6 kuukauden aikana.
- Hoito anabolisilla steroideilla tai kalsineuriinin estäjillä (siklosporiini, takrolimuusi)
- Päivittäinen hoito oraalisilla, intranasaalisilla tai inhaloitavilla glukokortikoideilla viimeisten 12 kuukauden aikana.
- Altistuminen mille tahansa tutkimuslääkkeelle 12 kuukauden sisällä.
- Yli 2 alkoholijuoman nauttiminen päivässä tai laittomien huumeiden käyttö 12 kuukauden sisällä seulonnasta.
- Ei sovellu tutkimukseen osallistumiseen muista syistä tutkijan harkinnan mukaan.

Knee Injury & Osteoarthritis Outcome Score (KOOS) JR

Patient reported knee function score.
The KOOS, JR was developed from the original long version of the Knee injury and Osteoarthritis Outcome Score (KOOS) survey using Rasch analysis.
The KOOS, JR contains 7 items from the original KOOS survey.
Items are coded from 0 to 4, none to extreme respectively.
KOOS, JR is scored by summing the raw response (range 0-28) and then converting it to an interval score (0-100).
The interval score ranges from 0 to 100 where 0 represents total knee disability and 100 represents perfect knee health.

Precision Error on Knee Bone Density Measurement

|
Precision assessment in the field of bone densitometry is the process whereby the ability of the instrument and the technologist to reproduce similar results, given no real biologic change, is tested.
To achieve statistical power, the investigators will take duplicate knee bone density measurements on 30 participants at either their 6 month or 15 month visit.
The standard deviation for each participant is calculated, then the root mean square standard deviation for the group is calculated.
The mathematical result of precision assessment is displayed as percent coefficient of variation (CV).
