소개
이 문서는 인간을 대상으로 한 연구를 위한 프로토콜입니다. 본 연구는 적용 가능한 연방 규정과 기관 연구 정책 및 절차에 따라 미국의 우수임상관리기준(Good Clinical Practice)에 따라 수행됩니다.
수술 기술의 수많은 발전과 현재 미국에서 매년 250,000건 이상의 시술이 수행되고 있음에도 불구하고 회전근개 수술 후 힘줄 치유 실패가 자주 발생하며 보고된 비율은 94%에 달합니다. 치유되지 않으면 지속적인 통증, 나쁜 결과, 재수술이 필요한 경우 사회에 상당한 경제적 부담을 초래할 수 있습니다. 60세 이상의 연령에서 열악한 힘줄 치유와 관련된 몇 가지 요인이 일반적으로 찢어짐이 발생하는 뼈 힘줄 경계면의 혈관 감소로 인한 중요한 위험 요소인 것으로 나타났습니다. 지난 수십 년 동안 고정을 개선하기 위해 수많은 기술이 고안되었지만 수리 부위의 생물학을 다루거나 향상시키기 위해 개발된 기술은 거의 없습니다. 로티움 나노섬유는 최근 FDA 승인 지지체(FDA 510(K) #K183236)로 동물 연구에서 뼈의 힘줄 치유를 개선하는 것으로 나타났습니다. 이는 세포외 기질을 모방하는 역할을 하며 치유 조직의 더 나은 조직 구조를 제공하는 복구 부위에 세포를 집중시키고 결합시키는 데 도움을 줍니다. 현재 연구의 목적은 지지체의 사용이 회전근개 치유를 크게 향상시키고 수술 전후 복구 실패 위험이 높은 환자의 근력을 향상시키는지 평가하는 것입니다.
배경 정보 및 과학적 근거
회전근개 파열은 노인 인구의 어깨 통증과 장애의 빈번한 원인입니다. 일반적으로 보존적 조치가 실패할 경우 수술이 권장되는 경우가 많습니다. 성공적인 임상 결과는 뼈에 대한 힘줄의 치유에 크게 좌우되는 것으로 느껴집니다. 지난 20년 동안 수많은 수술 및 기술 발전에도 불구하고 모든 수리가 치유되는 것은 아니며, 재파열 또는 치유 실패 등이 회전근개 수술과 관련된 가장 큰 합병증으로 남아 있습니다. 이는 결과적으로 사회에 막대한 경제적 부담을 안겨주고, 결과적으로 수술은 열악한 결과를 낳고 궁극적으로 자원을 낭비하게 됩니다.
실패 이유는 다양하지만, 나이가 들수록 강한 상관관계가 있는 것으로 나타났습니다. 회전근개 질환의 자연사에 대한 관찰 연구에서 50세 미만의 환자에서는 회전근개 파열이 거의 발생하지 않은 반면, 60세 이상의 환자에서는 통계적으로 유의하게 한쪽 및 양측 파열 발생률이 더 높았습니다. 나이가 들수록 힘줄의 본질적인 특성이 변경되어 강직, 혈관저하 및 힘줄 치유 생물학의 전반적인 손상을 초래하는 것으로 여겨집니다. 더욱이, 복구가 실패할 경우 일반적으로 수술 후 첫 4개월 이내에 복구합니다. 따라서 수술 직후 세포질을 증가시켜 복구 영역을 강화하는 방법은 전반적인 치유를 개선하고 실패를 줄일 수 있습니다.
