外科技術は数多く進歩し、現在米国では年間25万件以上の手術が行われているにもかかわらず、腱板手術後の腱治癒の失敗は頻繁に発生しており、その率は94%に達すると報告されています。 治癒しない場合は、再手術が必要な場合に持続的な痛み、不良転帰、および社会への多大な経済的負担を引き起こす可能性があります。 60 歳を超える年齢では、いくつかの要因が腱の治癒不良に関連しており、通常断裂が発生する骨腱界面の血管分布が減少するため、重大な危険因子であることが示されています。 過去数十年にわたり、固定を改善するために数多くの技術が考案されてきましたが、修復部位の生物学に対処したり強化したりする技術はほとんど開発されていません。 Rotium ナノファイバーは、動物実験で骨に対する腱の治癒を改善することが示されている、最近 FDA に承認された足場 (FDA 510(K) #K183236) です。 それは細胞外マトリックスを模倣するように機能し、修復部位で細胞を集中させて結合するのを助け、治癒組織のより良い組織構造を提供します。 現在の研究の目的は、周術期に修復が失敗するリスクが高い患者において、足場の使用が腱板の治癒を大幅に改善し、筋力を強化するかどうかを評価することです。
背景情報と科学的根拠
腱板断裂は、高齢者における肩の痛みや障害の原因となることがよくあります。 通常、保守的な対策が失敗した場合、手術が勧められることがよくあります。 臨床結果の成功は、腱の骨への治癒に大きく依存すると考えられています。 過去 20 年間にわたる数多くの外科的および技術的進歩にも関わらず、すべての修復が治癒するわけではなく、再断裂または治癒不全が発生し、依然として腱板手術に関連する最大の合併症となっています。 これにより、社会に多大な経済的負担が生じ、手術が実施されても最終的な結果は悪く、最終的には資源が浪費されることになる。
失敗の理由は多因子ですが、加齢と強い相関関係があります。 腱板疾患の自然史に関する観察研究では、50歳未満の患者では腱板断裂がほとんどないのに対し、60歳以上の患者では片側および両側断裂の発生率が統計的に有意に高かった。 年齢が進むと、腱の固有の特性が変化し、硬直、血管減少、および腱治癒の生物学的全体的な障害が引き起こされると考えられています。 さらに、修復が失敗した場合、通常は手術後最初の 4 か月以内に修復します。 したがって、手術直後に細胞密度を高めて修復領域を強化する手段は、全体的な治癒を改善し、失敗を減らす可能性があります。
