상세 설명
뇌졸중 생존자의 약 65%는 상지(UE) 기능에 장기적인 제한을 경험합니다. 특히 팔에 도달하는 움직임의 제한이 두드러지며 환자의 장애 수준과 밀접한 관련이 있습니다. 일상 생활 활동은 종종 UE를 포함하기 때문에 도달 및 파악 기술을 재교육하는 것은 완전한 삶의 질로 돌아가는 데 중요합니다. 그러나 UE 기능의 효과적인 재활에 필요한 훈련 매개변수는 알려져 있지 않습니다. 최근 증거에 따르면 훈련 중 고속 움직임이 만성 뇌졸중 환자의 팔 움직임을 개선하는 데 효과적입니다. 따라서 큰 오류를 생성하는 빠른 움직임은 피드포워드 컨트롤러의 복원을 촉진하여 만성 뇌졸중 환자의 팔 움직임과 UE 기능을 향상시킵니다. 소뇌는 모터 오류로부터 피드포워드 컨트롤러를 학습하는 데 관여하기 때문에 개선은 소뇌-피질 네트워크의 무결성에 비례합니다.
이중 맹검 준 무작위 통제 연구는 만성 뇌졸중 생존자에서 수행됩니다. 참가자는 속도 편향 교육 그룹 또는 용량 등가 정확도 편향 교육 그룹(대조군)에 배정되고 훈련된 직업 또는 물리 치료사로부터 1주 동안 4일 교육을 받게 됩니다. 행동, 근전도 및 MRI 데이터는 개입 전 2주, 개입 후 3일, 개입 후 1개월 이내에 수집됩니다.

설명
포함 기준:
- 허혈성 뇌경색 후 최소 6개월
- 21세 이상
- 마비된 UE를 움직일 수 있는 잔여 능력을 보여줍니다(상지 Fugl-Meyer 운동 점수 >20/66).
- 2단계 명령을 따를 수 있음(MMSE 테스트 8번째 항목)
- 몸통이 고정된 상태에서 5초 이내에 몸보다 25cm 앞선 거리에서 도움 없이 팔을 뻗는 동작을 수행할 수 있습니다.
- Modified Ashworth Score < 3으로 평가했을 때 경증/중등도의 경직을 나타내지 않음
제외 기준:
- 뇌졸중 이외의 모든 신경학적 진단
- 신경 병증과 같은 말초 운동 제한
- 마비 UE에 영향을 미치는 정형외과 장애
- 더 많은 영향을 받는 UE의 심한 통증 또는 감각/고유 수용성 장애
- 시각적 무시(Albert 테스트에서 선의 4% 이상이 교차되지 않은 상태로 남음).
- 소뇌에 직접적인 영향을 미치는 뇌졸중이 있었다
- MRI 스캐닝에 대한 금기 사항
- 상지 Fugl-Meyer 운동 점수 >58/66으로 대부분 해결된 손상
