Circa il 65% dei sopravvissuti all'ictus presenta limitazioni a lungo termine nelle funzioni degli arti superiori (UE). In particolare, le limitazioni nei movimenti di estensione del braccio sono importanti e fortemente correlate con i livelli di compromissione dei pazienti. Poiché le attività della vita quotidiana spesso coinvolgono gli UE, la riqualificazione delle capacità di raggiungere e afferrare è fondamentale per il ritorno a una piena qualità della vita. Tuttavia, i parametri di allenamento richiesti per un'efficace riabilitazione della funzione dell'UE non sono noti. Prove recenti suggeriscono che i movimenti ad alta velocità durante l'allenamento sono efficaci nel migliorare i movimenti del braccio negli individui con ictus cronico. Quindi, movimenti veloci che generano grandi errori, favorirebbero il ripristino dei controllori feedforward e quindi migliorano i movimenti del braccio e le funzioni dell'UE in individui con ictus cronico. Poiché il cervelletto è coinvolto nell'apprendimento dei controllori feedforward dagli errori motori, i miglioramenti sarebbero proporzionali all'integrità delle reti cerebellare-corticali.
Verrà condotto uno studio controllato quasi randomizzato in doppio cieco nei sopravvissuti cronici post-ictus. I partecipanti verranno assegnati al gruppo di formazione sulla distorsione della velocità o a un gruppo di formazione sulla distorsione della precisione equivalente alla dose (controllo) e riceveranno 4 giorni di formazione per un periodo di 1 settimana da un terapista del lavoro o fisico qualificato. I dati comportamentali, EMG e MRI verranno acquisiti entro due settimane prima, 3 giorni dopo e un mese dopo l'intervento.

Descrizione
Criterio di inclusione:
- Almeno 6 mesi dopo un ictus sopratentoriale ischemico
- Almeno 21 anni di età
- Mostra capacità residua di muovere l'UE paretico (punteggio motorio Fugl-Meyer dell'estremità superiore >20/66)
- In grado di seguire un comando in 2 fasi (8° elemento del test MMSE)
- In grado di eseguire un movimento di estensione del braccio non assistito di 25 cm davanti al corpo entro 5 secondi con contenimento del tronco
- Mostrare una spasticità non superiore a lieve/moderata valutata con un punteggio di Ashworth modificato <3
Criteri di esclusione:
- qualsiasi diagnosi neurologica diversa dall'ictus
- limitazioni del movimento periferico, come la neuropatia
- patologie ortopediche che interessano l'UE paretico
- forte dolore o compromissione sensoriale/propriocettiva nell'UE più colpita
- negligenza visiva (più del 4% delle linee non incrociate nel test di Albert).
- ha avuto un ictus che ha colpito direttamente il cervelletto
- eventuali controindicazioni alla risonanza magnetica
- menomazioni per lo più risolte con un punteggio motorio Fugl-Meyer dell'estremità superiore >58/66

Comparatore attivo: Allenamento di abilità motorie complesse basato sulla velocità
I partecipanti eseguiranno 400 movimenti complessi al giorno per 4 giorni per un periodo di una settimana.
Il compito richiede ai partecipanti di navigare con la mano attraverso una "traccia" proiettata sulla superficie di un tavolo con una larghezza di 5 cm.
I partecipanti ricevono un punteggio adattivo in base al loro tempo di movimento. .

Questo intervento si basa su recenti prove che i movimenti ad alta velocità durante l'allenamento sono efficaci nel migliorare i movimenti del braccio in soggetti con ictus cronico. I partecipanti saranno premiati per i movimenti eseguiti entro un breve lasso di tempo.

Il gruppo orientato all'accuratezza riceve un intervento dose-equivalente con enfasi sull'accuratezza.
La larghezza della traccia proiettata sul tavolo è più stretta (meno di 2 cm) e il punteggio adattivo ricevuto si basa sulla loro accuratezza nel dire entro il limite della traccia.
