Yksityiskohtainen kuvaus
Noin 65 % aivohalvauksesta selviytyneistä kokee pitkäaikaisia rajoituksia yläraajojen (UE) toiminnassa. Erityisesti rajoitukset käsivarteen ojentavissa liikkeissä ovat huomattavia ja korreloivat voimakkaasti potilaiden vammaisuuden kanssa. Koska jokapäiväiseen elämään liittyy usein UE:ita, tavoittavuus- ja käsittämistaitojen uudelleenkoulutus on ratkaisevan tärkeää palatakseen täyteen elämänlaatuun. Silti UE:n toiminnan tehokkaan kuntoutuksen edellyttämiä koulutusparametreja ei tunneta. Viimeaikaiset todisteet viittaavat siihen, että nopeat liikkeet harjoituksen aikana parantavat tehokkaasti käsivarsien liikkeitä kroonista aivohalvausta sairastavilla henkilöillä. Näin ollen suuria virheitä aiheuttavat nopeat liikkeet edistäisivät myötäkytkentäisten säätimien palautumista ja parantavat siten käsivarren liikkeitä ja UE-toimintoja kroonista aivohalvausta sairastavilla henkilöillä. Koska pikkuaivot osallistuvat myötäkytkentäisten säätimien oppimiseen motorisista virheistä, parannukset olisivat verrannollisia pikkuaivojen ja aivokuoren verkkojen eheyteen.
Kaksoissokkoutettu lähes satunnaistettu kontrolloitu tutkimus suoritetaan kroonisista aivohalvauksen jälkeisistä eloonjääneistä. Osallistujat määrätään joko nopeusharjoitusryhmään tai annosta vastaavaan tarkkuusharjoitusryhmään (kontrolli), ja he saavat 4 päivän koulutusta yhden viikon aikana koulutetulta toiminta- tai fysioterapeutilta. Käyttäytymis-, EMG- ja MRI-tiedot hankitaan kahden viikon sisällä ennen toimenpidettä, 3 päivää sen jälkeen ja kuukauden kuluessa toimenpiteen jälkeen.

Kuvaus
Sisällyttämiskriteerit:
- Vähintään 6 kuukautta iskeemisen supratentoriaalisen aivohalvauksen jälkeen
- Ikää vähintään 21 vuotta
- Jäljellä oleva kyky siirtää pareettista UE:ta (Yläraajojen Fugl-Meyerin moottoripisteet >20/66)
- Pystyy noudattamaan 2-vaiheista komentoa (8. kohta MMSE-testissä)
- Pystyy suorittamaan avustamattoman käsivarren liikkeen 25 cm kehon edellä 5 sekunnissa vartalon kiinnityksen kanssa
- Ei ylitä lievää/kohtalaista spastisuutta arvioituna modifioidulla Ashworth-pisteellä < 3
Poissulkemiskriteerit:
- kaikki muut neurologiset diagnoosit kuin aivohalvaus
- perifeeriset liikerajoitukset, kuten neuropatia
- pareettiseen UE:hen vaikuttavat ortopediset sairaudet
- voimakas kipu tai sensorinen/proprioseptiivinen heikkeneminen UE:ssa, johon se vaikuttaa enemmän
- visuaalinen laiminlyönti (yli 4 % viivoista jäi ylittämättä Albertin testissä).
- hänellä oli aivohalvaus, joka vaikutti suoraan pikkuaivoon
- mahdolliset magneettikuvauksen vasta-aiheet
- enimmäkseen korjaantui, kun yläraajojen Fugl-Meyerin moottoripiste oli >58/66

Active Comparator: Nopeuteen perustuva monimutkainen motoristen taitojen harjoittelu
Osallistujat suorittavat 400 monimutkaista liikettä päivässä 4 päivän aikana yhden viikon aikana.
Tehtävä edellyttää, että osallistujat navigoivat kätensä 5 cm leveän pöydän pinnalle projisoidun "radan" läpi.
Osallistujat saavat mukautuvat pisteet liikkumisajan perusteella. .

Tämä interventio perustuu viimeaikaiseen näyttöön siitä, että nopeat liikkeet harjoituksen aikana parantavat tehokkaasti käsien liikkeitä kroonista aivohalvausta sairastavilla henkilöillä. Osallistujat palkitaan lyhyessä ajassa tehdyistä liikkeistä.

Muut: Tarkkuuteen perustuva monimutkainen motoristen taitojen harjoittelu
Tarkkuusharhainen ryhmä saa annosta vastaavan intervention, jossa painotetaan tarkkuutta.
Pöydään projisoidun radan leveys on kapeampi (alle 2 cm) ja saadut mukautuvat pisteet perustuvat niiden tarkkuuteen eli radan rajojen sisällä.

Scores range from 0 to 57, with higher scores indicating better performance.
Positive changes reflect improvements in limb function.

The test consists of performing specific upper extremity movements, with each sub-task scored on a scale from 0 to 2, where a score of 2 indicates normal performance.
Scores range from 0 to 66, with higher scores indicating better performance.
Positive changes reflect improvements in motor function.

The Box and Block Test (BBT) measures unilateral gross manual dexterity.
The test involves moving, one by one, as many blocks as possible from one compartment of a box to an adjacent, identical compartment within 60 seconds.
Scores range from 0 to 150, with higher scores indicating better performance.
