Około 65% osób, które przeżyły udar, doświadcza długotrwałych ograniczeń funkcji kończyn górnych (UE). W szczególności ograniczenia w ruchach sięgających ramion są wyraźne i silnie korelują z poziomem upośledzenia pacjentów. Ponieważ czynności życia codziennego często wiążą się z UE, przekwalifikowanie umiejętności sięgania i chwytania ma kluczowe znaczenie dla powrotu do pełnej jakości życia. Nie są jednak znane parametry treningowe wymagane do skutecznej rehabilitacji funkcji UE. Ostatnie dowody sugerują, że szybkie ruchy podczas treningu skutecznie poprawiają ruchy ramion u osób z przewlekłym udarem mózgu. W związku z tym szybkie ruchy generujące duże błędy sprzyjałyby przywróceniu kontrolerów wyprzedzających, a tym samym poprawiają ruchy ramion i funkcje UE u osób z przewlekłym udarem. Ponieważ móżdżek jest zaangażowany w uczenie się kontrolerów ze sprzężeniem zwrotnym na podstawie błędów motorycznych, ulepszenia byłyby proporcjonalne do integralności sieci móżdżkowo-korowych.
Podwójnie ślepe, quasi-randomizowane, kontrolowane badanie zostanie przeprowadzone na przewlekłych osobach, które przeżyły udar. Uczestnicy zostaną przydzieleni do grupy trenującej z tendencją do pomiaru prędkości lub do grupy trenującej z dokładnością do równoważnej dawki (grupa kontrolna) i przejdą 4-dniowe szkolenie w okresie 1 tygodnia przez wyszkolonego terapeutę zajęciowego lub fizjoterapeutę. Dane behawioralne, EMG i MRI zostaną pozyskane w ciągu dwóch tygodni przed, 3 dni po interwencji i jeden miesiąc po interwencji.

Opis
Kryteria przyjęcia:
- Co najmniej 6 miesięcy po niedokrwiennym udarze nadnamiotowym
- Co najmniej 21 lat
- Wykazuje szczątkową zdolność do poruszania niedowładnym UE (punktacja motoryczna kończyny górnej w skali Fugl-Meyera >20/66)
- Potrafi wykonać dwuetapowe polecenie (ósma pozycja w teście MMSE)
- Zdolność do samodzielnego wykonania ruchu ramienia na odległość 25 cm przed ciałem w ciągu 5 sekund przy unieruchomieniu tułowia
- Wykazują spastyczność nie większą niż łagodną/umiarkowaną, ocenianą w zmodyfikowanej skali Ashwortha < 3
Kryteria wyłączenia:
- wszelkie diagnozy neurologiczne inne niż udar
- ograniczenia ruchu obwodowego, takie jak neuropatia
- schorzenia ortopedyczne wpływające na niedowład UE
- silny ból lub upośledzenie czuciowe/proprioceptywne w bardziej dotkniętym UE
- zaniedbanie wizualne (więcej niż 4% linii pozostawionych nieskrzyżowanych w teście Alberta).
- miał udar bezpośrednio wpływający na móżdżek
- wszelkie przeciwwskazania do badania MRI
- w większości ustąpiły upośledzenia z oceną motoryczną Fugl-Meyera kończyn górnych > 58/66

Uczestnicy będą wykonywać 400 skomplikowanych ruchów dziennie przez 4 dni w okresie jednego tygodnia.
Zadanie wymaga od uczestników poruszania ręką po „ścieżce” rzutowanej na powierzchnię stołu o szerokości 5 cm.
Uczestnicy otrzymują punktację adaptacyjną na podstawie ich czasu ruchu. .

Ta interwencja opiera się na niedawnych dowodach, że szybkie ruchy podczas treningu skutecznie poprawiają ruchy ramion u osób z przewlekłym udarem mózgu. Uczestnicy zostaną nagrodzeni za ruchy wykonane w krótkim czasie.

Grupa z tendencją do dokładności otrzymuje interwencję równoważną dawce z naciskiem na dokładność.
Szerokość toru wyświetlanego na stole jest węższa (mniej niż 2 cm), a otrzymana ocena adaptacyjna opiera się na ich dokładności, aby powiedzieć, że mieści się w granicach toru.
