효과적인 환자-제공자 의사소통을 통해 생체 기증자 신장 이식의 형평성 증대 (EPPComm)
이 임상 시험의 목표는 이식 평가 상담 중에 흑인과 백인 환자와 이식 제공자 간에 발생하는 의사소통을 이해하고 이러한 의사소통 요소와 환자 및 제공자 요인, 환자가 보고한 결과 및 생체 기증자 이식 결과(생존 기증자) 간의 관계를 평가하는 것입니다. 추천, 평가 및 이식. 우리는 이러한 결과를 사용하여 이식 제공자를 위한 의사소통 기술 교육 개발을 알리고 흑인 및 백인 환자의 생체 기증 신장 이식과 생체 기증자 이식 결과에 대한 제공자의 의사소통에 대한 교육의 영향을 테스트할 것입니다.

포함 기준:
- (1) 신장 이식 평가를 위해 Cooperman Barnabas Medical Center(CBMC) 또는 Temple University Hospital(TUH)에 출석
- (2) 영어로 말하세요
- (3) 자칭 흑인 또는 백인 민족이어야 합니다.
- (4) 18세 이상이어야 합니다.
다음과 같은 환자를 포함합니다:
- 이전에 신장 이식을 받은 적이 있다
- 영어 능력이 제한적인 환자, 통역사를 통해 의사와 대화하는 경우
- 실제로 CBMC 또는 TUHS가 아닌 외부 위성 위치에서 평가된 환자.
CBMC 및 TUH의 신장 이식 프로그램에 고용되어 이식에 대한 환자 평가에 적극적으로 참여하는 모든 이식 신장 전문의, 간호사 및 사회 복지사는 연구에 참여할 자격이 있습니다. 이식 상담의 디지털 오디오 녹음을 거부하는 제공자는 자격이 없는 것으로 간주됩니다.
제외 기준:
- (1) 말을 하지 않거나 다른 방법으로 서비스 제공자와 대화할 수 없습니다.
- (2) 이미 CBMC, TUHS 또는 다른 이식 센터에 신장 이식 목록이 등록되어 있는 경우
- (3) 영어 능력이 제한적이며 영어 이외의 언어로 의사와 직접 대화할 수 있습니다.
(1) 가족이나 친구로부터 신장 기증 제안을 받았거나 (2) 가족이나 친구에게 LD 역할을 고려하도록 명시적으로 요청한 환자는 제외되며, 디지털 오디오 녹음을 거부하는 환자는 제외됩니다. 이식 상담에도 참가할 수 없는 것으로 간주됩니다.
동의할 수 없는 성인, 아직 성인이 아닌 개인(유아, 아동, 청소년), 수감자, 영어를 이해하지 못하는 개인 등의 특수 집단은 제외됩니다.
신장 이식을 기다리는 동안 여성 환자가 임신할 가능성은 거의 없습니다. 단, 임산부는 제외하지 않습니다.

실험적: EPPCom 공급자 교육
매우 흥미롭고 상호작용적인 60분짜리 대면 의사소통 기술 훈련 프로그램이 이 프로젝트의 일환으로 개발될 것입니다.
참여하는 이식 제공자는 교육 참여 전(사전)과 이후(사후)에 간단한 온라인 설문조사를 완료합니다.

환자의 가치와 문화적 선호를 지원하면서 LDKT에 대해 환자를 교육하고 잠재적인 생체 기증자 검색을 촉진하기 위한 커리큘럼이 개발될 것입니다.
커리큘럼은 교훈적인 교육과 기술 기반 의사소통이라는 두 가지 구성 요소로 구성됩니다.
교훈적인 구성 요소는 흑인과 백인 환자에 대한 환자-제공자 의사소통 및 LDKT 프로세스 결과의 영향에 대한 증거를 제공합니다.
기술 기반 구성 요소는 LDKT에 대한 토론을 이끌어가는 의사소통 자신감을 구축합니다.
