Deze studie zal de haalbaarheid en effectiviteit bepalen van een 10-behandelingen elektro-acupunctuur (EA) programma bij patiënten met door chemotherapie geïnduceerde perifere neuropathie (CIPN). De onderzoekers veronderstellen dat EA een haalbare en effectieve therapie zal zijn voor CIPN.

Dit is een pilotstudie met één arm die de haalbaarheid en werkzaamheid van EA bij 20 proefpersonen met CIPN beoordeelt. Het primaire doel is de haalbaarheid van een EA-programma van 10 behandelingen bij proefpersonen met CIPN te bepalen. Haalbaarheid wordt gedefinieerd als ≥15 patiënten die ≥8 EA-behandelingen voltooien. Secundaire doelstellingen omvatten veranderingen in neuropathische pijn en levenskwaliteit voor en na EA-behandeling. Er zal ook een verkennende correlatieanalyse van mechanistische biomarkers worden uitgevoerd.

Inclusiecriteria:

  • Man of vrouw van 18 jaar of ouder
  • Heeft minimaal 3 maanden vóór de start van de EA-behandeling curatieve chemotherapie gekregen (bijv. paclitaxel, docetaxel, nab-paclitaxel, carboplatin, oxaliplatin, vinorelbine, ixabepilone of vincristine)
  • Aanhoudende perifere neuropathie van graad ≥2 in de vingers of tenen volgens de National Cancer Institute Common Terminology Criteria for Adverse Events (NCI CTCAE) v3.0
  • Eastern Cooperative Oncology Group prestatiestatus van ≤2 (zie paragraaf 13.3)
  • Bereid en in staat om schriftelijke geïnformeerde toestemming voor het onderzoek te verlenen

Exclusiecriteria:

  • Gedocumenteerde medische voorgeschiedenis van neuropathie als gevolg van zenuwcompressie (bijv. carpaletunnelsyndroom, radiculopathie of spinale stenose)
  • Ernstige stollingsstoornis of bloedaandoening, naar het oordeel van de behandelend arts
  • Aanwezigheid van cellulitis of andere huidinfectie of aandoening die plaatsing van acupunctuurnaalden in handen of voeten onmogelijk maakt
  • Diabetes, tenzij Hgb A1c <7,5%
  • Instabiele hartaandoening
  • Pacemaker
  • Metalen platen
  • Bekende psychiatrische stoornis die de samenwerking met de vereisten van het onderzoek zou belemmeren

Experimenteel: Elektro-acupunctuur (EA)
EA is een niet-farmacologische behandeling die traditionele acupunctuur combineert met elektrische stimulatie. EA wordt in totaal 10 behandelingen toegediend over een periode van 7 weken. EA wordt tweemaal per week uitgevoerd in weken 1-3 en vervolgens wekelijks in weken 4-7. Elke behandeling duurt ongeveer 30 minuten.
Elektro-acupunctuur (EA) is een niet-farmacologische behandeling die traditionele acupunctuur combineert met elektrische stimulatie en waarbij een kleine elektrische stroom tussen paren acupunctuurnaalden wordt geleid.

The change in neuropathic pain after a 10-treatment EA program in subjects with CIPN, was assessed by the Brief Pain Inventory Short Form (BPI-SF) at Baseline, 2-Week Post EA, and 3 Month Post EA. Used to evaluate the severity of a subject's pain and the impact of this pain on the subject's daily functioning. Pain and interference are rated on a 10-point scale (0 = no pain/interference and 10 = pain as bad as you can imagine/complete interference). Higher scores mean worse outcomes.

Change in Quality of Life After a 10-treatment EA Program

Assessed by the Total Outcome Index (TOI), including the Functional Assessment of Cancer Therapy/Gynecologic Oncology Group-Neurotoxicity (FACT/GOG-NTX) subscale at Baseline, 2-Week Post EA, and 3 Month Post EA. The Total Outcome Index (TOI) ranges from 0 to 100, with higher scores consistently indicating better outcomes. Items are rated on a 0-4 Likert scale and negatively worded items are reverse scored so that higher values always reflect better health status. The neurotoxicity subscale score is calculated by summing the scored responses to the neurotoxicity items (with standard prorating if some items are missing), and the TOI is calculated by summing the Physical Well-Being, Functional Well-Being, and Neurotoxicity subscale scores. Lower scores on either measure indicate worse symptoms or functioning, while higher scores reflect improvement or better quality of life related to neurotoxicity.
