Secundaire preventie van kleine subcorticale beroertes (SPS3) Trial
Het doel van deze studie is om te leren of een combinatie van plaatjesaggregatieremmers (aspirine en clopidogrel) effectiever is dan aspirine alleen voor de preventie van recidiverende beroerte en cognitieve achteruitgang, en of intensieve bloeddrukcontrole geassocieerd is met minder recidiverende beroertes en cognitieve achteruitgang.
Op 21 juli 2011 adviseerde de DSMB om de anti-bloedplaatjes-arm van de studie te beëindigen vanwege een onevenwicht tussen algehele en belangrijke niet-CZS hemorragische SAE's en totale sterfgevallen in de experimentele anti-bloedplaatjescombinatie van aspirine + clopidogrel en een tussentijdse statistische analyse die nutteloosheid aantoonde. in de onderzoeksarm tegen bloedplaatjes. Het werd aanbevolen dat patiënten de standaardzorg van aspirine-monotherapie voortzetten tot hun studieafsluitingsbezoek. Ook aanbevolen om de waterdrukarm voort te zetten en af te ronden volgens het protocol.

Een beroerte is schade aan de hersenen veroorzaakt door problemen in de bloedvaten. Beroertes veroorzaken vaak verlamming, verlies van gevoel en spraak en andere problemen. Een lacunaire of kleine subcorticale beroerte tast het binnenste deel van de hersenen aan en veroorzaakt kleine schade ter grootte van een "erwt" als gevolg van verstopping van kleine bloedvaten in de hersenen.
Deze multi-center studie zal 3000 deelnemers rekruteren (van wie 20 procent Latijns-Amerikaans is) om erachter te komen of het gebruik van aspirine en clopidogrel effectiever is dan het gebruik van alleen aspirine om terugkerende beroerte te voorkomen bij patiënten met lacunaire beroerte bevestigd door MRI, en of het verlagen van een de bloeddruk van de patiënt onder de gebruikelijke limieten zal ook helpen om terugkerende beroertes te voorkomen en het denkvermogen te behouden. Zowel aspirine als clopidogrel worden veel gebruikt voor bloedstolling en beroertepreventie. Onderzoekers zijn van plan om erachter te komen of het samen gebruiken van de medicijnen effectiever is dan alleen het gebruik van aspirine.
Deelnemers worden willekeurig toegewezen aan een van de 2 soorten behandeling: alleen aspirine of de combinatie van aspirine en clopidogrel. Bovendien worden deelnemers toegewezen aan een van de 2 groepen bloeddrukcontrole. Het verschil tussen de twee groepen is het streefniveau van de systolische bloeddruk - 130-149 of lager dan 130. Het doel van het bloeddrukaspect van dit onderzoek is om erachter te komen of het verlagen van de bloeddruk na een beroerte helpt om terugkerende beroertes te voorkomen en de cognitie te behouden.

Beschrijving
INCLUSIEF:
Kleine subcorticale ischemische beroerte of subcorticale TIA.
Inclusiecriteria zijn gebaseerd op TOAST-criteria aangevuld met vereiste MRI-gegevens. Aan alle volgende criteria moet worden voldaan:
- Een van de lacunaire klinische syndromen van een beroerte (overgenomen van Fisher) die > 24 uur duurde in de afgelopen 6 maanden
- Afwezigheid van tekenen of symptomen van corticale disfunctie zoals afasie, apraxie, agnosie, agrafie, homoniem gezichtsvelddefect, enz.
- Geen ipsilaterale cervicale halsslagaderstenose (≥50%) door een betrouwbare beeldvormende modaliteit uitgevoerd in een erkend laboratorium sinds de kwalificerende kleine subcorticale beroerte (S3), indien hemisferisch.
- Geen cardio-embolische bronnen met een groot risico die antistolling of andere specifieke therapie vereisen. Cardio-embolische bronnen met een klein risico zijn toegestaan als antistolling niet wordt voorgeschreven door de huisarts van de patiënt.
- Subcorticale TIA met overeenkomstige laesie op DWI.
- MRI-bewijs van S3: Aanwezigheid van een S3 (1,5 en 2 cm in diameter overeenkomend met de kwalificerende gebeurtenis op DWI; wanneer TIA, ADC-beeld moet laesie of T2/FLAIR (hyperintense laesies) bevestigen (vereist voor alle hersenstamgebeurtenissen) OF meerdere S3's op FLAIR/TI( <1,5 cm in diameter) (hypointense laesies) b. Afwezigheid van corticale beroerte en grote subcorticale beroerte (recent of op afstand).
UITSLUITING:
Om in aanmerking te komen voor deelname aan het onderzoek, mag de patiënt niet voldoen aan een van de onderstaande criteria:
- Slag uitschakelen (aangepaste Rankin-schaal kleiner dan of gelijk aan 4)
- Eerdere intracraniale bloeding (exclusief traumatisch) of hemorragische beroerte
- Leeftijd onder de 30 jaar
- Hoog risico op bloedingen (bijv. terugkerende GI- of GU-bloeding, actieve maagzweer, enz.)
- Verwachte behoefte aan langdurig gebruik van anticoagulantia (bijv. recidiverende DVT) of andere plaatjesaggregatieremmers
- Eerdere corticale beroerte (klinisch of door neuroimaging gediagnosticeerd), of eerdere corticale of retinale TIA
- Voorafgaande ipsilaterale halsslagader-endarteriëctomie
- Verminderde nierfunctie: GFR <40
- Intolerantie of contra-indicaties voor aspirine of clopidogrel (waaronder trombocytopenie, verlengde INR)
- Een score < 24 (gecorrigeerd voor leeftijd en opleiding) op het Folstein Mini Mental Status Examination
- Medische contra-indicatie voor MRI
- Zwangerschap of vrouwen in de vruchtbare leeftijd die geen effectieve anticonceptiemethode volgen
- Geografische of sociale factoren die deelname aan een studie onpraktisch maken
- Geïnformeerde toestemming niet kunnen of willen geven
- Zal waarschijnlijk niet therapietrouw zijn/niet bereid zijn om terug te keren voor frequente kliniekbezoeken
- Patiënten die gelijktijdig deelnemen aan een ander onderzoek met een onderzoeksgeneesmiddel of -apparaat
- Andere waarschijnlijke specifieke oorzaak van een beroerte (bijv. dissectie, vasculitis, protrombotische diathese, drugsmisbruik)
